  2. Performance Reward Grant 2009 How much is available for each Partnership? Total - £145,034.60 Plus 20% - £174,041.52 Minus 20% - £116,027.68

  3. Performance Reward Grant 2009 What is the Partnership being asked to come up with? The PMG are looking for proposals for projects based on a number of principles, including; ‘Additionality’ Innovation. Delivering wider Community Strategy outcomes Joint working

  4. Performance Reward Grant 2009 20% more or less SSP will approve final spend. Info will be needed on Current and projected performance against LAA targets; Other mainstream funding/resources available to the partnerships; and Innovation/cutting edge projects.

Performance Reward Grant 2009 LAA outcome areas Older people can successfully move home after a hospital stay People with long term conditions have the information and support they need to manage their LTC Carers are supported in their role Vulnerable people can live in settled accommodation People with low level mental illness get access to the help they need Vulnerable groups get the health services they need
