HOW THE SERVICE HAS DEVELOPED • New Intermediate Care Centre • Increase in intermediate care beds - now 21 • Increase in Therapy imput to both intermediate care and stroke beds • Development of Reablement Service • Increase capacity to all service areas. • Full integration including clear formal management structure. • Developments in intermediate care services for older people with mental health needs.

What Have We Achieved • New Intermediate Care facility • Integration in action. • Increase in Intermediate Care provision. • Intermediate Care Strategy identifying development up to 2007. • Establish as key service provision in relation to prevention of hospital admissions • and the facilitation of hospital discharge. • Establish joint management arrangements between Carter Bequest • and Intermediate Care Services. • Staff in the Service have completed the Foundation of Rehabilitation Training..

What Have We Achieved • Modern Matron appointment to work across Intermediate Care and Community Hospital. • Clinical Co-ordinator working in Intermediate Care and managing access to beds across • Intermediate Care and Community Hospital. • Working towards the development of joint rehabilitation services across • Intermediate Care and community hospital. • Established joint management (Service Leads Structure) across intermediate care • and the community hospital.

  6. Future Commissioning Priorities Middlesbrough Intermediate Care Centre • Middlesbrough Intermediate Care/Rehabilitation • Need to develop a Single Point of Access for Intermediate Care. • Improved access to service for Older People with Mental Health needs in line with HASCAS Project.

  7. Commissioning Priorities for Carter Bequest Hospital • Need for increased qualified staffing ratios in line with the Hurst Audit. • Increased dietetics input for patients on the stroke unit. • Increased podiatry linked to falls prevention.
