  4. Epidemiologi • Grave’s disease memiliki prevalensi sekitar 60-80% dari kejadian tirotoksikosis. Prevalensinya bervariasi pada tiap populasi, terutama bergantung pada asupan yodium. Penyakit ini timbul pada 2% wanita, namun hanya sepersepuluhnya saja pada pria. Oftalmopati Graves “NO SPECS” • Class 0: No signs or symptoms • Class 1: Only signs (limited to upper lid retraction and stare, with or without lid lag) • Class 2: Soft tissue involvement (oedema of conjunctivae and lids, conjunctival injection, etc.) • Class 3: Proptosis • Class 4: Extraocular muscle involvement (usually with diplopia) • Class 5: Corneal involvement (primarily due to lagophthalmos) • Class 6: Sight loss (due to optic nerve involvement) • Khurana AK.   8. Patofisiologi Oftalmopati • Inflamasi otot ekstraokular, yaitu adanya infiltrasi selular yang pleomorfik, berhubungan dengan peningkatan sekresi glikosaminoglikan dan imbibisi osmotik air. Otot-otot tersebut membesar hingga dapat mencapai 8 kali normal, lalu menekan nervus optikus. Degenerasi dari serat otot menyebabkan fibrosis, sehingga terjadi myopati restriktif dan diplopia. • Infiltrasi sel inflamasi, yaitu limfosit, sel plasma, makrofag, dan sel mast dari jaringan intersisial, lemak orbital, dan kelenjar lakrimal dengan penumpukan glikosaminoglikan dan retensi cairan. Hal ini menyebabkan volume orbital meningkat dan secara tidak langsung meningkatkan tekanan intraorbital yang menyebabkan retensi cairan berlebih. • Kanski JJ, Bowling B. Neuropati Optik • Penurunan visus, berkaitan dengan RAPD, desaturasi warna, dan penurunan kemampuan membedakan terang. • Gangguan lapang pandang dapat berupa sentral atau parasentral dan dapat pula terjadi bersamaan dengan defek bundel serat saraf.
