  2. What is HIRA? Hazards Identification & Risk Assessment It’s an electronic tool that been designed to help us carrying out Activity Risk Assessments (ARA) in standard method, replacing the `paper- passed` Activity Risk Assessment system & create a data base of risk assessments for all type of Hazardous activities / tasks.

  3. What is HIRA? This electronic tool (HIRA) is organized to allow the users to identify hazards under three categories: • 1) Location Hazards. • 2) Equipments Hazards. • 3) Activity Hazards. • The user can type the hazards & precautions or simply use the preloaded hazards that can be selected from the search engine and the side picker; consequently after selecting any particular hazard the required precautions for that particular hazard will appear on a side picker which allows the user to select all applicable controls and precautions.

  4. Who should be trained on HIRA? Compulsory Training: • HIRA Originator –Maintenance / Engineering Authority • HIRA Team Leader – Area coordinators / Senior Engineers / • HIRA Approving Authority - Superintendents / Area coordinators / Maintenance Section Head. • HSE Advisors Recommended: • Shift Controllers • Process Engineers • Inspection Engineers • Contractor Safety Eng. / Officer

Generic Roles and Responsibilities • HIRA Originators:Maintenance / Engineering / Contractors Engineers ( job Initiators ) • ARA TEAM LEADER • Operations coordinators • Project Coordinator or Section Heads • Maintenance Section Heads • ARA TEAM MEMBERSa)ARA Team Leaderb) HIRA Originatorc) HSE Advisor • d) Project Eng. or contractor HSE Eng. If they involved in the activity. • ARA Team Leader can invite any required members like:- Inspection Eng., Process Eng., Elect Eng., Civil Eng. Etc.

HIRA APPROVERIn side the plant:- • 1) Superintendents • 2) Operation Coordinators • Outside side the plant:- • Maintenance Section Heads • Project Managers • Note: Before the PTW meeting, HIRA approval will be completed and the team Members will sign the ARA. • HIRA APPROVER WHEN RESIDUAL RISKS ARE MEDUM OR HIGH • Division Manager • Operations Manager • Project/ Engineering Managers
