Policy Statement • This Mini Manual provides the administrative procedures and proof of care documentation required for Market Direct • Product that will have a Destination Maternity Brand Label exhibited in the garment. Destination Maternity Corp. is • committed to delivering quality merchandise to its customer. These guidelines are based on the mandate that all Adult • Maternity Apparel merchandise purchased by DM must have applicable proof of care provided and must meet all • applicable laws and regulatory requirements, in addition to ensuring compliance with Destination Maternity Corp. • Customer satisfaction and acceptability. Destination Maternity Corp. performance standards are used as a guideline for • the evaluation of it’s apparel product acceptability. The Proof of Care provided is reviewed against DM Import testing • standards. Any deviation from DM Standards will be discussed at the time proof of care is reviewed. • NOTE: The proof of care or any subsequent result does not limit Destination Maternity Corporations • rights or diminish or remove Buying representative or vendor’s responsibilities and obligations under • the Purchase Order Terms and Conditions. Proof Of Care Requirements • ACTION STEPS: • DMC Form Required: Proof Of Care:Market Direct Vendors must fill out the DMC Fabric Detail Sheet (Form is located on the DMC website) for each fabric that encompass the garment on order and being submitted for approval (example: self, combo, lining, trim etc.) *See Key Information Slide for how to navigate our website. • DMC Form Required: Care label: Fill out the DMC Care Label Form (form is located on the • DMC website:wwot.destcorp.com.*See Key Information Slide for how to navigate our website. • Information: Make sure all information on both forms are completed and signed by the appropriate party; Care Label form – must be signed by either the Head of Production or Testing and the Fabric Detail Sheet must be signed by the Mill or Fabric Supplier of the fabric. Failure to do so will result in rejection of testing documentation. • Submittal: Submit the completed DMC Market Direct Care form, Plus the DMC Market Direct Fabric Detail Sheet to the DMC Testing Associate: Endia Johnson for review and approval. *See last slide for contact information. • DMC Review: The Testing Associate will review the Fabric Detail Sheet + Care Form and will follow up with an approval or rejection within 48 hours. *DMC will allow Mill or Fabric Suppliers Fabric Detail Sheet as long as it provides the same fabric detail information as the DMC form. If the information provided is not sufficient vendor will be alerted and will be required to resubmit forms properly. • Modification: Based on the fabric and care information provided on the forms and the approved fit sample, Destination Maternity reserves the right to modify the suggested care wording in order to ensure customer satisfaction and safety. May not dictate any care wording DMC decides to modify. • Approvals: • Approved styles: Testing Associate will provide detailed approval status + approved care wording to be followed for the production care labels. *If vendor is set up in the DMC PLM system: the testing care approval will be shared in PLM. Otherwise testing PLM approval page will be e-mailed directly to vendor. • Rejected Styles: Testing Associate will provide detailed action steps required in order to successfully achieve approve proof of care. *Once approval is provided vendors may print their care labels accordingly.   4. Care Label Requirements • THINGS TO NOTE: • 1. The following testing information is provided for vendors on the DMC Website: wwot.destcorp.com • *Please refer to Key Information Slide for how to navigate our website. • *US Care Symbols: these must be used on all care labels in the proper order • *RN#: DM RN# 79913 must be exhibited on care labels • *French Translations: all English verbiage on care labels must have the appropriate French Translations. * Translations are on the web • *Market Vendor Testing Manual: This manual must be reviewed before • submitting styles for Care Label Approval. • *DMC Market Vendor Care Label Form: This form must be used for all market testing submitted for proof of care . • *DMC Fabric Detail Sheet: This form is available in the event the Fabric Detail Sheet provided by vendor is not sufficient. • 2. Any styles produced with a belt (faux leather, metal chain belt, faux Leather with metal buckle • etc.) a lead free certificate or lead test report must be provided in addition to the fabric detail sheet. • 3. All Belts must be stamped with Country Of Origin. If the belt cannot be stamped a small Country of Origin label • must be stitched or otherwise permanently and conspicuously attached. • 4. Any styles produced with jewelry such as a necklace attached – the same lead free/nickel free • certificate or lab test report must be provided in addition to the fabric detail sheet. Conclusion • BEST PRACTICES: • Read the Market Direct Mini Manual – Proof Of Care before proceeding to fill out care information. • Know your testing contact so that you can troubleshoot questions before proceeding. • Make sure the care information and forms provided to DMC are accurate, complete and • signed by the appropriate party or testing will be delayed. Vendors will be held • responsible for these delays. • Use the DMC website as necessary for forms and to reference the testing manual. • Send DMC Care information e mail with attachments to testing associate. Make sure the DMC style number and PO# are listed in the heading of the e-mail to ensure quick response. Conclusion
