  4. Safety and Health questionnaire included

  5. Safety and QA included in “technical approach” evaluation criteria

  6. Safety/QA plan part of SOW deliverable by vendor

  7. LSST shall abide by vendor safety policies within their facilities

  8. Vendor addresses safety in the review of interfaces as part of Phase A design activities

  9. The preliminary design phase includes a specific safety analysis and provide review of results

  10. Additional safety audit work is included in Final Design Review meeting

  11. Prior to Factory Acceptance Testing (FAT), the FAT plan is reviewed (which includes safety and hazard analysis

  12. Specification includes required compliance documentation and supporting information

  13. Specific safety requirements included

  14. Overall interlocks and safety requirements defined

  15. Safety and Health Audit Questionnaire
