  1. Children’s Therapy Program Children’s Therapy Program services are accessible to all families in the Sunrise Health Region for children from birth to age 5. Autism Services are provided to age 19. Working together…for healthy people in healthy communities.

Mission Statement • Children’s Therapy Program is dedicated to ensuring the availability, coordination and provision of preventative, rehabilitative and support services for families and children with special needs related to neurological deficits, musculoskeletal impairments, and developmental delays.   3. Individual Family Services -Supports Individual Family Service Plans by providing a coordinating link with CTP team and the family as well as community resources. -Provides information, supportive counseling and assistance to children and families dealing with the challenges of a disability.   4. Autism Spectrum Disorder Services Services for families and children from birth to age 19 who are concerned about or are diagnosed with ASD. Program includes screening for ASD, diagnostic support, individual autism intervention programs, links with other community services and supports. Autism Intervention Kerry Musey ASD Support Worker Brittany Bazansky ASD Support Worker Leahette Zerff ASD Support Worker ASD Support Workers have specialized training in autism intervention strategies. ASD Support Workers carry out individualized program plans developed by the Consultant for children up to age 5 who have been diagnosed or are suspect with an Autism Spectrum Disorder.   6. Early Childhood Psychology Provides developmental and psychological assessments, diagnoses, consultation and interventions for children from birth to school entry-age and their families. Eileen Deswiage R. Psych (provisional) Working together…for healthy people in healthy communities.

Occupational Therapy Birth to Kindergarten Entry -Inpatient OT services are provided to children while in hospital with a referral from attending physician. -Outpatient OT services are provided throughout the SHR with referral from parent/ legal guardian or health/ community service provider with parental consent. Specialized Services: Developmental Assessment, Assistive Technology Ages 6-18 years -one time assessments and follow up treatment for acute and post operative physical therapy. Physical Therapy Birth to Kindergarten Entry -Inpatient physical therapy services are provided to children while in hospital with a referral from the attending physician. -Outpatient PT services are provided throughout the SHR with referral from parent/ legal guardian or health/ community service provider with parental consent. Specialized services: Adaptive Equipment, Developmental Assessment, Specialized Seating, and Prosthetics Orthotics Ages 6-18 years -one time assessments and follow up treatment for acute and post operative physical therapy. Speech-Language Therapy Kory Bell SLP south Krista Herzog SLP central Lilia Livingstone SLP north Tasha Sudsbear SLP / ASD Kim Weichel SLP central Geri Koch SLP assistant -Provides diagnostic and programming services for children from birth to age five with language, communication and oral motor difficulties. -Specialized services such as feeding/swallowing and video flouroscopy are referred to Tertiary Clinics. Follow up treatment and management services are provided by CTP speech-language pathologists in consultation with the specialized service provider.   26. Services may be provided in one of our clinics, in child’s home, or a community setting to provide opportunities to develop and practice skills in the child’s natural environment. Working together…for healthy people in healthy communities.

  27. When referral is received… • An initial family meeting will be scheduled to determine areas of strengths and concerns • An assessment (formal or informal) is completed on all children Working together…for healthy people in healthy communities.

A therapy program and family service plan will be developed based on the child’s and family strengths, concerns, goals, and dreams! -may include direct therapy / group sessions -monitoring progress • Ongoing revision of goals and planning with changing needs and goals of child and family Working together…for healthy people in healthy communities.
