  1. KISD Dyslexia Web Application On KISD Connect

  2. When to Request Testing • You are encouraged to go through the RTI process • After a 504 or ARD meeting in which it is determined that a student should be tested for Dyslexia, go to the web application and request testing

  3. To log on to KISD Connect, go to the KISD website’s home page and click on KISD Connect. KISD Connect

  4. At the prompt, enter your NT user ID. KISD Connect Login Form

  5. Maintenance Links In the upper left-hand corner, you will see Maintenance Links and Staff Links. Staff Links should be expanded. Click on Maintenance Links to expand the list.

  6. Under Maintenance Links, click on Asssessment. Assessment

  7. Select A Report Default is “Select A Report” Dyslexia Web Pages

  8. Dyslexia Request for Testing (Contacts) Enter the student’s ID , click on Military Transfer radio button if necessary, and then click Submit.

  9. Request for Testing Input Screen At the top of the screen, please verify that you have the correct student. This is very important, as the wrong student has been entered on the log in the past.

  10. General Data Input The year defaults to the current school year. The grade defaults to the student’s current grade. • Choose the appropriate test language. • Click on ARD or 504 as appropriate. • Enter the Parent Permission Date. You should not request testing until you have the parent’s permission to test and obtained vision and hearing tests.

  11. Vision and Hearing Vision and hearing tests should be completed before you request testing. A student should have passed both hearing and vision tests, or have a doctor’s note stating that it is okay to continue testing the student. All dates default to today’s date, and should be changed to reflect the actual date of the test.

  12. Vision Data Please indicate if a students needs glasses or contacts for testing. Please ensure that the student has the appropriate corrective lenses for the day of testing. (Give reminders)

  13. Hearing Data Please indicate if a students needs hearing aids for testing. Please ensure that the student has hearing aids if needed for the day of testing. (Give reminders)

  14. Dyslexia Log (Contacts & Assessors) The Dyslexia Campus Log screen shows a list of the students tested for your campus. Notice the new icons under the Update and Profile columns.

Assessment Status All Requests New Hold Screen Appeal Committee Complete Normal Flow of Dyslexia Assessment Status New – contact initiates request and a record with a ‘New’ status is created Screen – assessor changes status to ‘Screen’ while testing, then to ‘Committee’ when testing is done Active Requests New Hold Screen Appeal Committee – ARD or 504 committee meets and determines if student qualifies For Dyslexia Web App issues, call Renee Ham x0320 renee.ham@killeenisd.org Complete – after committee meeting, contact changes status ‘Complete’

  17. Dyslexia Campus Log The Dyslexia Campus Log screen shows a list of the students tested for your campus. Assessor Statuses: New – New Requests Screen – Assessors are Testing Committee – Tested and Ready for ARD or 504 Meeeting Student Statuses: Qualified Does Not Qualify Monitor Lab (no longer used) Date: The date of the decision After the ARD or 504 meeting, you will want to update the student’s status.

  18. Dyslexia Student Info Update The data that was entered on the “Dyslexia Request for Testing” web page may updated. This data is highlighted in light blue.

  19. Dyslexia Test Results Update Status Information, Assessment Information and Test Results may be updated here. Shown here are the Status and Assessment fields.

  21. Dyslexia Profile Click on the Dyslexia Profile link to view the profile in Adobe PDF. You may print the profile or save and email as needed. Here, the name, ID and birthdate are blank for privacy purposes. If you see some ‘missing’ data, especially for data that is over 2 years old, please contact your assessor and have them pull the hard-copy of the profile and update the database.

  22. Dyslexia Profile Scores This profile may be saved as a PDF and emailed to other districts requesting the information.
