  2. Service Provider Information • Applications: you only need to apply once; your application will be automatically renewed each quarter. If you no longer want to be a notetaker for our office, you need to notify us in writing. • Identifying the consumer(s): Students who receive services through our office have the right to completeconfidentiality. • Confidential information includes but is not limited to: consumer(s) name(s), disability of student(s), and/or specific information related to the class. Please keep this in mind when trying to identify the consumer(s) at the beginning of each term.

  3. Notetaker Information • First week: By the end of the first week of your assignment, you are responsible for leaving a note in the students box stating when (day and time) you will have notes to them. Make a copy of this note and give it to the Accommodations Coordinator. • Now what?: Take notes to the very best of your ability. Write down examples that the professor says or writes on the board, write down questions the students ask, and the answers the professor provides. Please be sure to write down ALL of the information provided in the class lecture.

  4. Responsibilities • As the notetaker, you must have neat, legible handwriting and must be able to keep pace with the lecture. You must also arrive on time to class; professional behavior is a must at all times! • In order to ensure a quality product, please submit a copy of your notes by the end of the first week to the accommodations coordinator for feedback. • The Office of Disability Services will provide a photocopy machine for you to make the appropriate copies.Please respect the fact that the use of the copy machine is for ODS purposes only; you may not make personal copies, or copies of your notes here for non-ODS students.

  5. What do I do when… • I can’t make it to class?: If you are unable to attend class for any reason you are still responsible for obtaining the notes from another student and making the appropriate copies.

  6. How do I get paid? • At the end of the term you must submit a invoice along with your yellow time log to ODS. Invoices are placed in your mail folder during week 10. • You will be paid $20 per credit hour per term. However, if you, or the student you are taking notes for, drops the class partway through the term or if you do not fulfill your responsibilities, your pay will be pro-rated. Upon receipt of your invoice and time log we will process your paper work. This could take approximately 3-4 weeks once the term is complete. • Note: Classes with labs are broken down according to their lecture and lab time; i.e., some five credit classes are three hours of lecture and two hours of lab. More often than not, notetaking is NOT provided for labs. Therefore, unless otherwise assigned, payment is made for the lecture portion only.

  7. Helpful hints for taking notes • Be on time to class. • Sit near the front of the classroom. • Write legibly, someone else has to be able to read and study from your notes. • Use a black ink pen. Pencil tends to smear and is harder to read. • Highlight class instructions, homework assignments, due dates and announcements, important information and key words. You can use circles, check marks asterisks, underlining etc. to indicate important information. • Define vocabulary and use instructors’ examples to clarify concepts. • Use diagrams and illustrations effectively when applicable. • Use correct spelling. If you are unsure, write “sp” and try to correct it later. • Leave plenty of blank space to make it easier to organize ideas. • An outline format is helpful to organize information and makes it easier for the consumer to navigate through the notes. • Please take notes on only one side of the paper. • Take notes on all films shown in class.

  8. Don’t forget your header! The header is REQUIRED! In the top right-hand corner of the first page: • Your name • Today’s date • Name of course (i.e, HST 201) On the following pages, always indicate date and page number

  9. To do list… • Put a note in the student’s box indicating when you will have their notes turned in to them. • Turn in a copy of your notes to the accommodations coordinator by the end of the first week for review. • Go to class daily, take great notes and get them turned in everyday after class. • Have a great quarter!
