4. 4 Objective of the Expert Patient Program To improve the understanding that patients with heart failure have on their disease by sharing knowledge with an expert patient and other patients with the final aim of improving quality of life and acceptance of the disease.
5. 5 Specific Objectives of the Expert Patient Program Provide information on signs and symptoms of the disease with particular emphasis on signs of alarm.
Increase the knowledge of patients on precipitating factors of decompensation.
Increase the involvement of patients in the management of their disease and prevention of complications.
Increase patient’s self-confidence and skills
6. 6 Who is an “Expert Patient”... ? An “expert patient“ (EP) is a person with a chronic disease whose knowledge and profile enables him/her to lead a group of patients with the same disease and who is capable of:
Taking an active part in managing their own care
Knowing and identifying the signs and/or symptoms of their disease and responding to them
Acquiring tools that help in managing the physical, emotional and social impact of the disease

Explanation of the Program to health care professionals
Informative sessions for patients of the Center
Selection of the Expert Patient
Constitution of the patient group
Theoretic content and methods for the Expert Patient given by health care professionals
Theoretical/practical group sessions led by the Expert Patient
Support and feedback
8. 8 Expert Patient Program Groups of 10 patients + Expert Patient.
Nine sessions over two months.
Duration of each session 1.30 hours.
Theoretical and practical sessions.
Training, support in leading and feedback for the expert patient.

Signs of alarm and decompensation

The results obtained in the Barcelona Pilot Program are encouraging and in agreement with those obtained by the National Health Service in England.
The implication of the people involved in the Program has been very satisfactory.
