Employee Referral Program • The purpose of the Employee Referral Award Program is to encourage Agfa Graphics, Agfa HealthCare, Agfa Materials and Agfa Finance (“Agfa”) employees to refer qualified individuals for employment opportunities with Agfa. • Agfa employees may earn an Employee Referral Award for referring applicants for open positions in Agfa. Awards are subject to all required withholding deductions. There is no limit on the number of Employee Referral Awards that an employee may earn. • The actual award amount for each qualified Agfa job opening will generally be specified in the 'Employee Referral' section of each position's job posting located on the Agfa Corporate Intranet. • The award will be paid provided: • The individual referred is on the payroll as a regular full-time employee • The referring employee is still an active employee when the award is processed. • The award will be paid within the first 90 days of the referred applicant's start date.

  36. Safety Principles • All injuries and occupational illnesses can be prevented. Zero incidents is a reasonable goal and is the target for Agfa Corporation • Management is directly responsible for preventing injuries and illnesses, with each level accountable to the one above and responsible for all levels below. This applies to all levels, from the President to the newest employee • Safe behavior is a condition of employment for all • Creating a safe work environment for employees is a good business decision and adds to overall business performance. Safety is a critical part of job performance

  37. Safety Principles (continued) • Safety awareness will be raised through appropriate training, communication, written standards and procedures, and audits. Corrective actions will be taken in response to deficiencies • Continuous improvement will be made in safety performance. Goals and measures will be established • All incidents that may have resulted in harm to employees will be investigated and evaluated • Individual responsibility and accountability for personal safety is critical. All employees will be involved in safety activities • Safe behavior will be reinforced through positive feedback and deficiencies will be responded to with appropriate and consistent disciplinary action

What to do if injured at work • Seek EMERGENCY treatment immediately • Remember, first responsibility is to your immediate health – get assistance • Advise the hospital/emergency room that the injury is work related • If injured at an Agfa site (facility), follow local procedure • Notify and report immediate facts to your Supervisor • Inform your Human Resources Manager • You have a right to any of your own work related medical records. If you would like information on your own medical records, contact your Human Resources Manager

Emergency Evacuation Procedure • Your first responsibility is to evacuate and warn others • The basics are always the same if an alarm is given: • Shut down any hazardous equipment if easy to do and quick • Leave your area and do not bring anything but your wallet/purse, car keys, and coat (if needed and easily at hand) • Close the door to any room you are in behind you • Leave by the nearest safe exit and go to your assembly area
