  3. Window Opening • The Year 5 E-rate window will open on Monday, Nov. 5, 2001 • It will close on Thursday, Jan. 17, 2002 • Forms 470 must be posted on the SLD web site by Thursday, Dec. 20, 2001 to allow for the 28-day posting requirement Michigan Information Network

Form 486 • You must complete a separate Form 486 if: • You receive FCDLs featuring more than one entity number • The Item 11a certification applies to certain FRNs and the Item 11b certification applies to other FRNs (for Funding Y4 and later) • You file a waiver of CIPA requirements for a portion of your funded FRNs (for Y5 +) Michigan Information Network

  5. Form 486 cont. • When to file? • No earlier than the receipt of an FCDL • Funding Years 1, 2, and 3 • Should file within 10 days of the start of services • Funding Year 4 and later • Covered in the “CIPA Filing Requirements for Year 4” session Michigan Information Network

  6. Form 486 Notification Letter • New letter currently under development by the SLD to confirm receipt of your Form 486 We have received your Form 486! Michigan Information Network

  7. Form 500 • Is being updated due to CIPA requirements • Cannot be used to change the Service Start Date to a date earlier than what is on the Form 486 Notification Letter Michigan Information Network

SPIN Changes • Post Copan requirements are more flexible • Applicant has to make 3 certifications: • Change is allowed under state & local procurement rules/laws • Change is allowed under terms of contract • Original Service Provider (SP) has been notified of intent to make change Michigan Information Network

  11. SPIN Change Process • Applicant sends letter to: • SPIN Change Request Schools & Libraries Division Correspondence Unit P.O. Box 125 80 South Jefferson Road Whippany, NJ 07981 Michigan Information Network

SPIN Change Process cont. • Applicant and original SP are given telephone notice of the change • September 2001 - SLD to institute e-mail notice • New SP receives FCDL • September 2001 - SLD to provide more accurate information about amount remaining to be disbursed Michigan Information Network

  13. Product and Service Substitutions • Substitution of the products and services indicated on Form 471 is allowed in limited circumstances • Recent changes provide greater flexibility Michigan Information Network

  17. Service Substitution Procedure • Submit service substitutions as “minor modification” via Block 2 of Form 471 • Attach letter with detailed description of “from” and “to” for proposed changes Michigan Information Network

Service Substitution Procedure cont. • Attach certification described on web site (Reference Area - Service Substitutions) • Submit at least 90 days prior to the last day to receive service in order for PIA to have sufficient time to review and notify SP and Applicant Michigan Information Network

E-certification • SLD working to implement for Year 5 Form 471 window opening • Will require applicant to apply for User ID and PIN • SLD will no longer require “Description of Service” attachments to be filed before the window closing (but they still must be filed!) Appeals - Tips for Successful Filing • Appeal must be received within 30 days of the date of the decision letter • Provide Application Number, FRN, etc. that is under appeal • Explain what decision you are appealing • Explain why you think the SLD made a mistake • Provide the backup documentation Michigan Information Network

FCC Deadline Extensions • Deadline for delivery and installation of nonrecurring services is the September 30 that follows the June 30 close of the relevant funding year • If certain “decisions” (e.g., FCDL SP change approved) are issued after March 1, nonrecurring services may be delivered through September 30 of the following calendar year Michigan Information Network

Top 10 Reasons for Funding Denials • 1) Missed filing deadlines • 2) 28-day rule (Know your ACD!) • 3) No Form 470 posted • 4) Competitive Bidding (vendors can’t do it for you) • Applicants must consider all bids • Price must be predominant factor in selecting a bid Michigan Information Network
