Why? • Our healthcare system is rapidly changing as Health Homes and Managed Care become reality. • Optimize possibilities by forming a partnership to implement a workforce retraining initiative. • Case Manager’s job expectations have changed. • Assure that case managers learn the competencies they need to meet the demands of change by utilizing web based and face to face classes. • Assure that existing case managers gain the tools needed to provide care coordination within our new Health Home environment.

Unprecedented Pace of Change • October 2011: Managed care plans took over the Medicaid pharmacy benefit • January 2012: Regional Behavioral Health Organizations began efforts to improve hospital discharge and community services follow up plans for hospitalized ‘high needs’ individuals • January-June 2012: Medicaid beneficiaries are being assigned to new coordinated Health Home networks • 2013-4: Medicaid mental health, substance use and medical services are put into some form of fully capitated managed care system.

New York State Care Management Training Initiative (NYSCMTI) • NYSCMTI is contributing to the successful transformation of the health/behavioral health care delivery system. • NYSCMTI prepares OMH case managers and OASAS MATS staff to deliver quality, integrated care coordination services to Health Home members. • NYSCMTI has designed a dynamic package of face-to-face and web-based training opportunities to help staff succeed in their new roles as Health Home Care Coordinators.

The Training Package • The Trainer: New York Care Coordination Program (NYCCP), a nationally recognized leader in the delivery of evidence-based training and technical assistance services develops, coordinates and delivers all trainings. • Over a two year period we will provide nine 90 minute webinars to introduce new skills and enhance existing competencies staff need to succeed in their new roles. • Two (2) day-long face-to-face trainings will be held across New York where participants can acquire new information and practice newly learned skills. • Full access to aNYSCMTI website stocked with articles, videos and learning tools enabling trainees to engage in self-directed learning between scheduled trainings

Trainee Requirements • Full Trainee: Completes a pre-assessment survey online, views “live” webinars, and participates in face-to-face workshops. Commitment to the entire training series is required- attendance is tracked - this is how continued funding is assured. • Alternate Trainee: Completes a pre-assessment survey online and can view recorded webinars. Is an “alternate” for face-to-face workshops. • Both full and alternate participants receive user ID & password in order to attend trainings.
