MassMATCH MassMATCH is Massachusetts’ initiative to Maximize Assistive Technology (AT) in Consumers’ Hands, a federally-funded statewide AT program to promote the use of AT devices and services to enhance the independence of people with disabilities, enabling equal participation in all of life's activities.   13. Waiver Services Waivers allow states to provide a broader range of services to specific populations than are generally covered through regular Medicaid coverage. Massachusetts’ waivers vary in the specific services they offer, but some of the waiver services include: Adult Companion Personal Care Behavioral Supports and Consultation Respite Day Services Shared Living 24 Hour Supports Home Accessibility Adaptations Specialized Medical Equipment Individual Support & Community Transitional Assistance Habilitation Services Peer Counseling/Peer Support Vehicle Modifications

  14. Money Follows the Person Demonstration and Waivers

Money Follows the PersonPopulations Served • Elders 65 + • Adults with disabilities age 18 – 64 • People with intellectual and developmental disabilities • People with physical disabilities including acquired brain injury • People with mental illness (cannot be sole disorder or disabling condition)

Money Follows the Person:Qualification Criteria for Demonstration • Residing in an inpatient facility for 90 or more consecutive days (excluding Medicare-funded rehab days) • Nursing facility • Chronic or rehabilitation hospitals • State psychiatric hospitals (for specific age group) • ICF/MR • Resident of Massachusetts • Eligible for Medicaid + at least 1 Medicaid-paid inpatient day • Consented to the program • Transitioning to an eligible residence • Home / apartment owned or leased • Community-based residential setting

  17. MFP: Demonstration MFP includes both a Demonstration program and Waivers. The Demonstration assists consumers to leave institutions and follows them for the first 365 days that they are in the community. Demonstration participants may or may not enter a waiver, and can apply for a waiver after moving to the community. Under the Demonstration, a Transition Entity determines eligibility for the Demonstration, enrolls consumers and provides Transitional Assistance goods and services if needed. All Demonstration services end after the 365 days. Most ILCs and ASAPs are Transition Entities.

MFP Demonstration Participants MFP Demonstration Participants may access services through the MassHealth State Plan (including One Care, SCO, PACE or access to other State Plan services) or through the following HCBS waivers: • Frail Elder Waiver • MFP Community Living Waiver • MFP Residential Supports Waiver • ABI Residential Habilitation Waiver • ABI Non-Residential Habilitation Waiver • DDS Intensive Supports Waiver • DDS Community Living Waiver • DDS Adult Supports Waiver

  19. MFP Demonstration Services MFP Demonstration services are available for 365 days after the individual transitions to the community. These services include: • Demo Case Management • Demo Assistive Technology • Orientation & Mobility Services • Transitional Assistance Services

  20. MFP: Waiver Applications The MFP waivers are separate from enrollment in the MFP Demonstration. To apply for a waiver, submit an application to U. Mass. Call 855-499-5109 for information. Both the ABI and MFP Waivers will be open for new applicants shortly; ABI on April 1, 2015 and MFP on May 1, 2015. Consumers can begin submitting applications at any time.
