The Patient-Centered Medical Home Defined Personal physician- first contact, continuous and comprehensive care Physician directed medical practice - team provides ongoing patient care Whole person orientation - provides for all patients’ health care needs or arranges care with other professionals. Includes care at all stages of life (acute, chronic care and end-of-life care and preventive services) Care is coordinated and/or integrated - across health care system (e.g., subspecialty care, hospitals, home health agencies, nursing homes) and the patient’s community. Care facilitated by registries, information technology, health information exchange. Care provided when and where needed.

The Patient-Centered Medical Home • ASO and Provider Service Networks • Provider Service Networks are community focused • Allows for more $ to be used for the actual provision of care • (medical loss ratio) • Creating a single network or organization that cares for nearly all of patients needs • Theorectically should result in stabilizing costs • Current primary care networks serve as a gateway to the model
