Why Might a Person Who Wants to Self-Direct Choose Family Care? -Support from an interdisciplinary team -Case management -RN involvement -IDT staff expertise and knowledge of community resources -Support from Family Care MCOs -MCOs have staff with specialized expertise (e.g. Durable Medical Equipment)

  4. Why Might a Person Who Wants to Self-Direct Choose Family Care? -May choose to self-direct some supports but not others -Access to a provider network -Flexibility to change between self-directed supports and contracted providers

  5. SDS: Opportunity for creativity • Services other than Supportive Home Care • Within managed care, SDS is most commonly used to hire employees to provide hands-on assistance, cleaning, respite, etc. There are other applications as well. • SDS does not always equate to being an employer • Non-conventional therapies • Work place personal assistance

SDS: Opportunity for creativity • Transportation debit card example • Have more choices with services • Offers more flexibility • Maximizes member involvement -Things for teams to consider • Is the member able to handle independently, or is assistance needed? • Needs to be monitored closely • MCO may restrict level of self management under certain conditions (last resort)
