Assessment • Assess prerequisite knowledge (Form A) • Use Diagnostic Test from the previous grade- diagnose readiness by testing content that should have already been learned • Use Diagnostic Test from current grade- pretest on content you plan to teach • Summative Evaluation (Form B) • Use Diagnostic Test of current grade after providing instruction or intervention- choose the test or part of a test that assesses content taught Form A Form B

  3. Assessments • Grade Specific • Organized by booklet topics • Parallel Forms (Form A and Form B)

Diagnosis • Use Class Record Form to analyze test results(find form in Math Diagnosis and Intervention System Teacher’s Guide or online Forms and Correlations) • Mark the form you are using • Mark items missed • Write the total number correct- note scores that fall below the proficiency level (66%)

  5. 3. Intervention • Use grade specific intervention lessons for intervention on prerequisite skills at the start of the year, topic, or lesson. • Intervention after lessons- Use for intervention on content (based on needs of individual students or groups of students)

  6. 3. Intervention • Grades K-2 • Interventions lessons consist of an activity (teacher directed) and problems • Grade 3 • Students answer a series of questions that guide him/her to the correct answer and additional (similar) problems

Monitoring • Use Individual Record Form to track progress (find form in Math Diagnosis and Intervention System Teacher’s Guide or online Forms and Correlations) • Mark the Test Form used • Circle items missed on the assessment • Write the total number correct for each part of the assessment • Circle the intervention lessons you use • Communicate- use this form to communicate with parents during conferences or with other teachers that are working with the student Use Individual Recording Form to see which Intervention Lesson to use.
