  3. Day 1 Out Patient appointment – allow to try voluntary treatment despite history – named person feeling they can cope Out Patient notes make clear that if any further concerns should be detained

  4. Day 1 Restricted information relating to police involvement / child protection information from another local authority

  5. Day 20 Concerns reported by Children and Families Team GP advising client to go to hospital A+E informed patient that they could be brought back by police Police returning client to hospital with police escort

  6. Day 20 Duty doctor following information on TRAK to detain Visit to named person by DASAT (Domestic Abuse and Sexual Assault team, Police)

  7. Day 21 Phone Call from police (Public Protection Unit / DASAT) and Team Manager Mental Health Phone call from RMO to duty MHO for assessment under Section 44 MH(C+T)(S)A Phone call to Named person to request their views re possible detention under Section 44

  8. Day 25 Appropriateness of named person Information provided by named person confirmed significant risk to others

  9. Day 26 Phone call from police – no locus for further involvement Day 32 Meeting with client including questioning him regarding information obtained from named person

  10. Day 40 Mental Health Tribunal Hearing under Section 50 (Patient’s right to apply for revocation of short-term detention certificate ) Application not upheld

  11. Day 42 Threats of harm made by client to female nursing staff Day 43 CTO application completed including information from named person

  12. Day 52 Named person has legal representation – named person saying they had not requested this Preparation for tribunal

  13. Day 53 Mental Health Tribunal – request by named person’s lawyer for interim order – named person present Interim CTO hospital based granted Information re past history from client’s lawyer

  14. Day 54 Discussion with RMO and AWI and MHO manager re application under rule 68 (Excluding persons from hearings in exceptional circumstances)

  15. Day 57 Named person reporting client no longer making statements that would support risk to named person Day 62 Discussion with named person’s lawyer – not willing to continue acting if change of instruction

  16. Day 76 Mental Health Tribunal hospital based CTO granted Major faux pas – not checking source of evidence – not in nursing notes

  17. Day 112 Named person reporting unhappiness at any discussion of client’s past behaviour Day 127 Named person reporting that she would inform staff that client’s passes home were going well even if they were not

  18. Day 144 Client discovered to have been going home without agreement of RMO Named person not reporting this to ward staff
