  17. Many people currently potentially misdiagnosed... Worse if only studies around diagnostic threshold used: sensitivity of 86% and specificity of 46%

  19. What threshold ABPM ? Based on Head et al BMJ 2010 • adjust by 5/5 mmHg at lower threshold (stage 1 hypertension, 140/90 mmHg clinic) • ie < 135/85 mm Hg • 10/5 mmHg at higher threshold (stage 2 hypertension, 160/100 mmHg clinic) • Ie < 150/95 mmHg

  30. How does this translate to recommendations?

  31. Diagnosing hypertension (1) If the clinic blood pressure is 140/90 mmHg or higher, offer ambulatory blood pressure monitoring (ABPM) to confirm the diagnosis of hypertension

  32. Diagnosing hypertension (2) When using the following to confirm diagnosis ensure: ABPM: • two measurements per hour during the person’s usual waking hours (Day time mean) HBPM: • two consecutive seated measurements, at least 1 minute apart • blood pressure is recorded twice a day and for at least 4 days • measurements on the first day are discarded – average value of all remaining is used

  35. Conclusions • Diagnosis of Hypertension is changing • Implementation challenging but benefits can be realised in terms of better targeting of treatment and reduced costs
