The AFIX Product Functionalityin ImmPact • Perform provider level immunization rate assessments • Assessments are a key part of the Immunization Quality Improvement for Providers (IQIP), an initiative that aims to provide continuous quality improvement measures to help providers strive for and achieve higher immunization rates, as well as improve immunization practices.   6. Accessing the AFIX Product To navigate to “the afix product” in ImmPact, go to the “Reports menu group” and click the link for “the afix product” Maine Center for Disease Control and Prevention

  7. The AFIX Cohort There are three AFIX Cohort assessment types: • Childhood • Adolescent • And Combined The default landing page is the AFIX Cohort page. The outline of the AFIX Cohort menu item will be highlighted to indicate current selection from the menu panel. Maine Center for Disease Control and Prevention

  8. Childhood Criteria The specific assessment measures and corresponding age range will display both Assessment Criteria measures and age ranges on the cohort screen. Maine Center for Disease Control and Prevention

  11. As of Date Select the desired “As of Date” or use the default value of the current date. You may specify a different date by typing the date, formatted as MM/DD/YYYY, or by using the calendar option. Maine Center for Disease Control and Prevention

  12. Birthdate Range Based on the value selected for the ‘As of Date’ input field, the system will auto-populate the Birthdate range that fall within the appropriate age range for the selected assessments. Maine Center for Disease Control and Prevention

Types of Reports The following options are: • Assessment • Patient Listing • Provider Report Card • Invalid Doses (excluding Rotavirus) • Missed Opportunities (excluding Rotavirus) • Missed Immunizations (excluding Rotavirus) Choose the radio button(s) to the left of the desired report name and click the “Submit” button. Assessment • This option will create a report based on the specified assessment criteria requested. • For childhood assessments, the patient compliance with the specified AFIX measurements is assessed at 24 months/2nd birthday. • For adolescent assessments, the patient compliance with the specified measurements is assessed on the date of assessment. • The assessment report will represent the coverage percentages for the selected provider in both graphical and tabular format. Additional details indicating the numerator and denominator for each measure will also be provided in the report. Maine Center for Disease Control and Prevention

  17. Patient Listing The ‘Patient Listing’ option will create a report listing of the patient(s) assessed. It will provide specific information on whether the patient(s) listed were considered up- to- date (UTD) for the measures assessed for the specific childhood or adolescent cohort measures. Each Childhood or Adolescent Cohort will display each antigen in separate columns. Maine Center for Disease Control and Prevention

Invalid Doses • The ‘Invalid Doses’ option will create a report listing of the patient’s assessed and the antigen(s) for which the dose(s) were considered invalid. • **Note: Invalid doses will only display in the report results if the patient is NOT UTD on the As of Date for the selected report criteria. Missed Immunizations The ‘Missing Immunizations’ report component identifies and lists patients in a cohort that were indicated as Not Up-to-Date (UTD) and missing immunizations at the time of the assessment. The missing immunization(s) displayed under the ‘Antigen Missing (Doses) column will display the dose number in the series missed as (#). Maine Center for Disease Control and Prevention

Flexible Cohort • In addition to the age specific Childhood and Adolescent assessments on the AFIX Cohort page, the AFIX Product also provides a custom assessment by using the Flexible Cohort page. • To schedule a Flexible Cohort Assessment, click the ‘Flexible Cohort’ from the left menu tab.   26. Flexible Cohort Assessment Parameters The flexible cohort screen provides a set of parameter options to run assessment type reports based on either a specific date or by a specific age. As of Date The ‘As of Date’ option sets the base date used to calculate the ‘From’ and ‘Through’ Birthdate ranges. When selecting the ‘As of Date’ option on from the Assessment Parameters, The AFIX Product will capture all immunization activity up to and including the ‘As of Date’ for patients whose birthdates fall within the Birthdate ‘From’ and ‘Through’ ranges. The Month and Day (MM/DD) values of the ‘As of Date’ are used to set the ‘From’ and ‘Through’ dates of the Birthdate Ranges. The ‘As of Date’ is set as the default selection.   27. As of Birthdate When selecting the ‘As of Birthday’ option a user will be required to enter a specific age (in years). Based on the age entered, The AFIX Product will only include immunization activity up to and the day they turn the age (year) selected (e.g., up to and including 13th birthday). This is different from the ‘As of Date’ option which captures immunization activity up to a specific date. When selecting the ‘As of Birthday’ the Age Range values can only be calculated in years.   28. Age Range The age input boxes accept only numeric characters with the exception of zero. The numeric characters entered in the ‘From and Through’ fields may be the same or in ascending order. The Age Range values entered will set the Years (YYYY) for the ‘From and Through’ Birthdate Ranges. Continue the selection using the picklist to indicate Months or Years when ‘As of Date’ option is selected. Maine Center for Disease Control and Prevention

Vaccine Group andCorresponding Dose Count • Select the vaccine group from the drop down box and the corresponding dose number needed for the assessment and click the ‘+’ sign to add the selected vaccine to the table. • The Vaccine Group Selection options are as follows: Maine Center for Disease Control and Prevention

  30. Dose Count The Dose Count box displays dose count values as 1 – 6 doses. If a user selects ‘HPV’ from the Vaccine Group Selection then the Dose Count list will display values as 1 – 6 doses and ‘UTD’. The ‘UTD’ dose count selection will not display for any other vaccine group other than ‘HPV’. Maine Center for Disease Control and Prevention

HPV Note: If the Vaccine Group ‘HPV’ and the Dose Count ‘UTD’ are selected The AFIX Product will count UTD based on Series complete for either 2 dose (2 of 2) or 3 dose (3 of 3) series rather than at looking at the dose count. If the HPV series is not complete (1 of 2) or (2 of 3) the patient will not be counted as ‘UTD’ for the UTD-HPV selection. All other antigens selected from the Vaccine Group selection list will continue dose count calculations based on dose level (1 – 6) selected. Maine Center for Disease Control and Prevention

The Birthdate Range Based on the values selected for the ‘As of Date’ input field the system will auto-populate the Birthdate range ‘From and Through’ fields that fall within the appropriate age range for the selected Childhood, Adolescent or Flexible assessments.   33. Flexible Reports • Select the radio button(s) to the left of the output report desired and click the “Submit” button. The available output options are: • Assessment • Patient Listing • Invalid Doses • Missed Opportunities • Missing Immunizations Maine Center for Disease Control and Prevention

Flexible Assessment • This option will create a report based on the specified assessment criteria requested. • The Flexible Assessment captures assessments based on: As of Date As of Age Birthday and the Age Ranges • The assessment report will represent, in both graphical and tabular format, the coverage percentages for the selected provider.   35. Flexible Patient Listing • The Flexible ‘Patient Listing’ report will create a report listing of the patients assessed. It will provide specific information on whether the patients listed were considered up- to- date (UTD) for the measures assessed for the specific childhood or adolescent cohort measures. The ‘Dose Count Completed’ column value will display as either ‘Y’ or ‘N’. If the ‘Dose Count Completed’ column value displays as ‘Y’ the patient is considered up to date based on the antigen and number of doses selected from the Vaccine Dose Selection option. Flexible Invalid Doses • The ‘Invalid Doses’ option will create a report listing of the patient(s) assessed and the antigen(s) for which the dose(s) were considered invalid. • *Note: Invalid doses will only display in the report results if the patient is NOT UTD on the As of Date for the selected report criteria. Flexible Missed Opportunities • The ‘Missed Opportunities’ option will create a report listing of the patient’s assessed and the antigen(s) for which the dose(s) were considered missed as of the date a last vaccine was administered. • A missed opportunity is defined as a vaccine that could have been administered at the same time the of the last immunization date. Flexible Missing Immunizations • The ‘Missing Immunizations’ report component identifies and lists patients in a cohort that were indicated as Not Up-to-Date (UTD) and missing immunizations at the time of the assessment. The missing immunization(s) displayed under the ‘Antigen Missing’ (Doses) column will display the dose number in the series missed as (#). • The report looks at the same age range as childhood assessment report (24 – 35 months). • If a vaccine is not given prior to 2nd year birthdate it will be considered missed. (Note: Childhood Missing Immunizations excludes Rotavirus) • The Missing Immunizations list includes patient name, patient date of birth, missing vaccine type and dose number missed. Clear and Submit Buttons • Using the “Clear” button will clear the screen, and you may start over again using a different set of criteria. • When the ‘Submit’ button is clicked, a message will display at the top of the page to indicate that the job has been scheduled successfully, displaying the Job ID number associated with the scheduled job.   40. List of Scheduled Jobs The list of scheduled jobs displays a listing of all jobs scheduled by users from both the AFIX and Flexible Cohort pages. Maine Center for Disease Control and Prevention

More on List of Scheduled Jobs • The default display is for jobs submitted within a month, but the functionality exists on the page to search jobs by providing date ranges. • The Search button can be used when changing the ‘Jobs Submitted Between’ and ‘Through’ date ranges from those currently displayed to expand or narrow search results. The Search button also acts as a ‘refresh’ button to update the current view with the date range remaining unchanged. • The Advance Search button may be used to select specific criteria from each of the job details. (With the exception if Job ID which is an exclusive search and will select a single record for display). All other fields may be used in any combination, with one or all options uses.   42. System Monitor • The System Monitor page serves as a dashboard to indicate the status of the various components of AFIX Product. Access to the page and buttons presented are based on the role of the user. • When the technical manager clicks on the failed jobs button, a popup is displayed, and the header Failed Jobs displays, with both “Jobs Submitted Between”, “Through” fields and a “Search” button. Failed Messages • When the “Failed Messages” button is clicked, the technical manager is presented with a table containing 9 columns: Job ID, User ID, Provider, Submitted On, As of Date, Job Type, Report Type, Internal Status and Download Files.
