Kenosha Community Health Center • Our mission is to: • Provide healthcare access to underserved in Kenosha County • Reduce Health Disparities • 2 locations • Medical 4536 22nd Ave. • 2 Family Practice physicians, 1.2 pediatric physicians, .2 Obstetrician, 3 Physician Assistants • Dental 6226 14th Ave • 14 Full time dentists, 7 Hygienist • In 2009, 41,000 patient visits, serving approximately 13,000 individuals • Pediatric patient visits – 7500 ( 2009 )

  3. Quality Initative • KCHC, as a federally qualified health plan participates in over 40 quality initiatives • Financial • Operational • Clinical • Immunization Quality Initiative

  4. Immunization Quality initiative • 2 Goals: • Meet the immunization standards of State of Wisconsin. • Increase immunization rate of KCHC from 54% to target 85% by 2014.

  5. Quality improvement projects Increase Newborn follow up rates Vaccinate all Pediatric patients seen at KCHC.

  6. Vaccination Criteria Follow guidelines published by AAP ( American Academy of Pediatrics). Vaccinate Health checks/Preventive visits AND sick visits.

  7. Newborn follow up Follow up conducted in accordance to Bright future guidelines of AAP. Follow up from 2 weeks of age to 24 months. Patients name noted and follow up call given 2 weeks prior to scheduled health check.
