  1. 14th May 2012Evaluating a clients health and thinking about SMART targets for devising a health care plan. LO: To self assess our questionnaires to ensure they meet the appropriate pass criteria to assess an individuals health needs. To understand what is meant by SMART targets and use them to help devise a health care plan. Objectives • Assess our health questionnaires to ensure they address the key health factors: physical, socioeconomic, health and lifestyle • To think about how to create a care plan according to SMART Targets to meet the health and wellbeing needs of an individual • To identify key ways to assess an individuals health needs

  3. Factors We can categorise the factors which influence our needs. These categories are • Socio Economic • Physical • Lifestyle • Health Check list: You should have a completed P2 Task which clearly explains what each of these factors are with a range of examples for each • You now know all the factors that have an impact on health and wellbeing.

  4. Assessing an individuals general health and wellbeing • In order to produce a health plan you will need to think of a persons PIES and their health needs linked to physical ,socio economic, lifestyle and health factors. Case study - P 64/56 book • We are going to read over the 3 case studies and then you are going to explain ( in writing & vocally) how a persons age, preferences and lifestyle influence a health plan. • You must also be able to explain why confidentiality is important when you collect information from or about an individual. Task : Are you Thinking ?

Self Assessment Now check if your questionnaire covers all the key questions that need to be asked to assess someone's health: You may have to change some of your questions!! • Physical– age , height/ weight, family history (genetics), environment • Lifestyle– smoking, diet, exercise, alcohol/drugs, sexual practices, personal hygiene. • Health– illness, injury, disability, mental health. • Socio Economic – Employment, culture, income, education, living conditions. You cannot ask rude or invasive questions eg How much do you earn? Do you have good personal hygiene or what social class are you?

  6. P3 Assessing Needs – P 66/67 Physical and physiological measurements – what is the difference? Physical is to do with the body eg height and weight whereas physiological is linked to the functions of the body eg blood pressure, pulse rate or cholesterol levels. Independent Task: You have 15 mins to complete the 3 questions in the Just checking box

Homework – due next week • You must now carry out an assessment of the health and wellbeing of an individual you know using your amended Questionnaire thus why your questionnaire must cover all factors. • Have you included questions which you can use to assess on heart/ cardiovascular issues/ BMI/ mobility/ fitness/Mood & self esteem/ diet/asthma/ diabetes etc • This can be done with a family member or a friend including someone making up the data by pretending to be a particular patient that requires a care plan eg someone with asthma, high blood pressure, diabetes, heart problems etc

  8. Care Planning • Care plans are used to enable an individual to access the help that they need and become independent. The aim of a care plan is to enable a service user to gain empowerment over their own lives and develop a strong sense of self worth and a high self esteem.

  9. Smart Targets (P 68/69) • When creating a care plan, targets have to be set and evaluated. • Targets must be SMART • What does this mean?

  10. Task 3 Use the pages of your book (p68/69) to demonstrate your understanding of “Smart Targets” (P3) Assessment) Draw a diagram to show you understand what is meant by smart targets

Next week: Assessment Task P4 • In your groups, use the information that you have collected in the questionnaire, write a statement outlining the individual’s everyday health and well being needs. • Go on to produce a care plan for your service user, this should include 3 needs and targets with short, medium and long term interventions. • There should also be a column to indicate the evaluation that will take place to ensure that the needs and targets are met.

  12. Extension M1/D1Care Plan Analysis • Make a neat copy of your care plan. • Answer the following question in a word document.. • Explain how the achieving the targets will meet the individuals health needs and wellbeing (PIES). • How does the plan take in to account the individuals personal circumstances and preferences, give examples. • Explain the possible difficulties that the individual may encounter and suggest how these will be overcome.
