  9. PROFILAXIA SECUNDARĂ Ciroză fără terapie profilactică primară Ciroză cu sângerare sub terapie cu βblocanţi Contraindicaţii sau intoleranţă la βblocanţi

Eşecul profilaxiei secundare • Un singur episod semnificativ clinic de resângerare -  cu 3g a Hb, fără transfuzii - ABRI  0,5 - hematemeză, melenă - peste 100 ml sânge pe sonda naso-gastrică • Opţiuni terapeutice- TIPS sau şunturi splenorenale distale în insuficienţa hepatică clasa A/B- TIPS poate rămâne singura opţiune în insuficienţa hepatică severă secundară hemoragiei variceale-TIPS poate fi punte pentru transplant în insuficienţa hepatică

  11. TRATAMENTUL HEMORAGIEI PRIN VARICE GASTRICE Clasificare:- iniţiale - secundare Clasificarea SARIN: gastroesofagiene tip 1 (joncţionale) gastroesofagiene tip 2 gastrice izolate tip 1 gastrice izolate tip 2 (ectopice)

  12. Profilaxia primară • β blocante neselective • NU tratament endoscopic

  13. Tratamentul hemoragiei prin varice gastrice - rolul benefic al substanţelor active nu a fost probat (dar se administrează!) • scleroterapia • ligatură şi ansele detaşabile • tamponament cu sonda Blakemore • TIPS – eşec al terapiei endoscopice • embolizare retrogradă asociată ocluziei cu balonaş • tratament chirurgical
