This a plan that the big bosses in health and social care have to fill in every year to tell the government if we are providing good care and services to people with a learning disability locally Lots of people from health and social care are working on the plan As part of the work we need to ask people with a learning disability and their families about their experiences of using health services We have to tell the big bosses what we find out by the end of July Learning Disability Self Assessment Framework for 2013 and 2014

  2. Compliance- policies plans following the law Data- information about numbers Measures- Checking how well areas are doing Lived experience- Peoples real stories These are the big things the bosses in health and social care want to know about this year

  3. “I am supported to maintain my independence for as long as possible” “ I understand how my care and support works, and what my entitlements and responsibilities are” " I am happy with the quality of my care and support” “I know that the person giving me care and support will treat me with dignity and respect” I am in control of my care and support Using the ‘I statements….’

  4. The big bosses have asked the commissioners, (these are the people who plan and buy services for people with learning disabilities) to find out about peoples lives and their experiences to help them plan good services for the future How do we find out about peoples experiences?

  5. We would like to plan a day where we can meet people on the 28th of June so they can tell us their stories and real life experiences. We need to ask people with learning disabilities, their families, advocates, providers (people who run services) and others to meet with us We will then tell the Commissioners what people say to help them plan and buy services for the future We will also tell the Clinical Commissioning Groups and the Health and Well Being Boards what people say Lived experience- my story
