  1. Common Assessment Framework and Self Assessment Questionnaire The Common Assessment Framework that was talked about earlier in the year is being tried out for some people in the council and health. The council is still using the Self Assessment Questionnaire as well and still filling out its own assessment forms.

  2. Update on the common Assessment Framework • We have now been told by our Legal Department that it meets all our legal requirements • The form has now been put onto Care First 6 • The learning Disability Teams are having their training on Care First 6 and once they have had it they will be able to use the CAF • This should be achieved by the end of November 09

  3. Also, in the Council lots of different forms are used for different people so if you are an older person it is a different form or if you have a mental health need. • The Council will be supporting everybody to have an individual budget in the future.

  4. The Council would like to have one Self Assessment Questionnaire that is used by health and the Council. • The questionnaire would need to be flexible and able to show everybody’s needs. • The questionnaire would become the main way for people to work out their health or social care budget.

  5. The Council have not started work on this yet. • The Council would want customers to be involved in helping them to write any new Self Assessment Questionnaire

  6. This is not definite yet. • The Council would like to know what the Board think. • There are 2 questions • 1. Is one Self Assessment Questionnaire a good idea? • 2. Should customers be involved in writing it?
