Key Information • Complete entire health packet as soon as possible- find it online at pacific.edu/healthservices.xml • Eligibility: registered for current semester and paid the $120 health fee (fee cannot be waived if enrolled in 9 or more units or if living on campus) • Staff: • 5 Nurse Practitioners • Consulting Physician • 3 Medical Assistants • Registered Dietitian • Administrative staff

University Insurance Requirements • All students enrolled in 9 or more units must have comprehensive medical insurance • If an HMO, coverage within 100 miles of campus • Deductible cannot exceed $2,500 • Online waiver must be completed at pacific.edu/insuranceoffice.xml by September7th to waive off the University sponsored plan • Must submit a copy of the health insurance card to the Insurance Office for verification by September 7th • Waiver process must be completed each Fall or the student will not get a refund

University Health Insurance: Anthem Blue Cross PPO • $789/semester for: 8/1/11-1/31/12 and 2/1/12-7/31/12 • Health Center: 100% coverage for services, $10 Rx co-pay • In-network off-campus care (referral required within 30 miles of campus): • 80% coverage for most services, 100% coverage for preventive care • $100 annual deductible, waived for preventive care and office visits • Prescription coverage: $10 generics, $20 brand, $30 non-formulary • Vision coverage included, go to pacific.edu/insuranceoffice.xml for benefit details • Coverage is applicable when student is home, on vacation, or studying abroad

Preparing for the Real World • Students should learn tomanage their own health concerns • Make appointments • Understand appointment etiquette • Understand their health insurance coverage • Complete own forms and waivers • Pay bills in a timely manner • HIPAA & FERPA • Records are kept confidential
