DFG Filing Fee Cash Receipt All portions of this form are required to be completed with the following information: • The Lead Agency, normally the “From” party. • County/State Agency of Filing. • Document No. • The Project Title. • The Project Applicant, sometimes referred to as “Agency or Person carrying out project”. • The Project Applicant Address, this is a complete address including zip code. • A Contact Phone Number. • You must be able to identify what entity the project Applicant is, • Local Public Agency • School District • Other Special District • State Agency • Private Entity

  5. CEQA No Effect Determination Form • If there is a CEQA No Effect Determination form attached to the document, then the filing is exempt from fees. • A photocopy of the CEQA No Effect Determination form is acceptable.
