About CO Care Tablet

Objective

Help older adults with multiple chronic conditions manage their care during transitions.

B. Goals

Develop a PHR application that allows older adults during transitions of care to:

  • Support self management

  • Coordinate care among different doctors

  • Prepare for possible urgent needs

  • Find assistance after return home

C. Current Model

We are modeling a proven paper PHR used during transitions of care.

The paper PHR allows for communication between older adult, coach, caregivers, and family.

Use off-the-shelf technology that will simplify input and communication.

Idea

Identify familiar paradigms and iteratively develop a PHR application for older adults that is “just at hand.”

Initial interviews show participants rely heavily on calendars for information.

Participant can scan medication information and receive reminders, information, etc. on the CO Care Tablet.
