  4. 4 Basic Classifications of Disciplinary Action • Employee counseling or oral reprimand • Written Reprimand • Suspension • Dismissal

  5. Disciplinary Action Criteria • The employee is counseled by the supervisor following an offense. • All disciplinary actions are documented. • Effect of the incident on the record is 12 months. • Return to work policy due to safety related issues.

  6. The purpose of this section is to outline the specific disciplinary guidelines for safety related issues and return-to-work policy. Zero Tolerance Policy Safety related non-compliance issues shall be addressed with a progressive disciplinary procedure as outlined below: 1. Verbal Warning First offense of a severity 1 incident, Effect of the incident on the record is 12 months 2. Written Reprimand If the sum of the severity of the incidents on record adds up to 2 or a Severity 2, Effect of the incident on the record is 12 months 3. Time off If the sum of the severity of the incidents on record adds up to 3 or a Severity 3 Effect of the incident on the record is 12 months 4. Dismissal If the sum of the severity of the incidents on record adds up to 7 or more A Severity 4 violation

Zero Tolerance Return-To-Work Policy • Away from employment for 30 days • Recommended by field supervision for rehire • Appear before Safety Task Force Review Board • Complete a “Commitment to Safe Behavior” form • On probation for a 90 day period, with any non-compliance safety issue resulting in dismissal • Placed in the “At-Risk Employee Program” – 90 day
