  3. Student Verification Includes personal information such as your address, phone number, emergency contact, etc. Parents need to make any corrections necessary and sign it. You need to return the signed form whether or not there are changes. You must send a current utility bill (water, electric, or gas) with this form. A bill with a cut off notice will not be accepted.

  4. Course Selection Form Everyone is on the College Prep track unless your parent signs an opt-out form. You do not need to take AP or Pre-AP classes to be on the college track. Required Courses Circle one on each line; ex: English I or Pre-AP English I. You cannot take a sport, PE, or Weightlifting at the same time. Electives Fill in each blank box with an elective. If the elective is a full year course, fill in the box with that course name for 1st semester and 2nd semester; ex: Spanish I is a full year so list it on the left and right. Alternative Course You must choose three alternate courses in case the ones you chose above are full. If you do not and a course is full we will have to choose a course for you and you will not be able to change it. Signature You and your parent must sign. By signing you understand that the choices you made on this form are the courses you will be taking and will not be changed in once school starts in the fall.

  5. DO NOT put Concurrent or Moore/Norman Technology classes on your course selection form. All six/seven classes you select will be at Moore High School. In August if you have been selected to attend Moore/Norman Tech or enroll in concurrent classes we will change your schedule at that time. Some of you will be placed in Algebra I enrichment or Geometry enrichment, this is based on your previous years grades. Any student reading two or more grade levels below his/her grade is required to enroll in reading. FYI—Course Selection Form

  6. Courses selected during course selection should be considered final. When students return the course selection forms and other documents, his/ her counselor enters his/her course requests into the computer. Then, for 4 to 8 weeks, the master schedule is formed from the list of requests of our approximately 2100 students. In August, students pick up their schedules at orientation and are allowed the opportunity to request changes, at that time. • Schedule changes will be given for the following reasons: • Credit has already been received for the course. • The student wants a more advanced course. • There is an open hour on the schedule. • The student does not have the pre-requisite for the course. • The student has previously failed with the teacher. • The student did not choose the course (provided course selection forms were returned by due date) • The student has been approved to attend Moore Norman Technology Center. • The student is approved for a concurrent course. • Once school starts for the semester, if a change is needed for one of the above reasons, the student will ask the teacher, whose class he/she needs changed, to complete a priority schedule change form. The teacher will give this form to the counselor’s office. Due to the high volume of schedule changes, these requests must be made in writing on the correct form. Schedule changes will be made in the order of priority, if approved. Some schedule changes will also require written permission from a parent. • Additionally, changes for second semester schedules may be requested via a counselor at any point during September through December. • Schedule changes will not be made after the first ten school days of the semester.   7. Suggested electives are more math, science, social studies, computers, world languages, courses dealing with your possible career choice. Some of the full-year courses have a number for 1st semester and a different number for 2nd semester. Some have one number for all year. If a course has a pre-requisite it will be listed. This means a subject must be taken before a student is allowed to enroll in another subject. If a course has a “#” beside it then it may be repeated for credit. You must have 5 credits to be considered 10th grade, 10 for 11th, 16 for 12th, and you need 23 to graduate.
