  5. Provincial Partners The Manitoba Health Experience CDPI Project Summary This project evolved out of a willingness of the partners to work towards maximizing health gains for all Manitobans. Project Goals • To provide a community-led, evidence- based approach to primary prevention of the chronic diseases that are major contributors to mortality in Manitoba (cardiovascular disease, cancer, diabetes, lung and renal diseases). Project Goals • To implement a sustainable chronic disease prevention initiative that draws upon strong partnerships. • To add value to existing initiatives, by integrating them with the CDPI. • To improve the health of Manitobans by reducing health disparities. The club meets once a week for a group walk which made it easier for me to make friends and a chance to get active and socialize at the same time. I find that I have more energy to look after my family. Being in the Walking Club, I have received numerous personal invites to attend local educational sessions, which I find very encouraging.
