What the data is telling us Not currently on our action plan – need to do some work here Taking Penwith as an example: [Insight Profile] Higher older population relative to rest of county/UK (28%) Higher than UK average proportion of adults on mental health related benefits Obesity is the most prevalent long term condition – nearly 20% of one practice’s population is obese Diabetes is the second most prevalent condition [ICA Databook] Higher than county average for physiotherapy referrals Higher than county average for trauma & orthopaedic elective inpatient referrals Overall referral rate is 6% lower than county average
