Faith, Family & Community Support Service Partnerships MHSA Innovation Projects MHSA INNOVATION projects are novel, creative and/or ingenious mental health practices or approaches that contribute to learning rather than focusing providing service. Innovation 6 Project – Faith Based Re-Entry Initiative for Newly Released County Inmates and the formation of a Faith Collaborative. Faith Collaborative Primary Purpose: • Engage and support re-entry individuals & family members in a natural community setting • Enable individuals to take care of themselves – promoting self-sustainability • Resources marshaled will support- • - Individual and basic life needs • - Recovery/Rebuilding lives Outcomes • Reduce recidivism • Improve quality of life • Increase faith connections 2

VISION, MISSION & VALUES • Mission: The Faith Re-Entry Collaborative of Santa Clara County is an inclusive faith-centered network offering hope, compassion, forgiveness, trust, and accountability together with immediate and long-term resources and supports to individuals and families as they return to the community from incarceration. • Collaborative Members agree to: • The mission, vision & purpose • Be a welcoming community • Collaborate with member congregations to meet felt and spiritual needs • Active participation in the Collaborative activities including: meetings, workgroups, and trainings • Include current services and resources in Faith Re-Entry Collaborative Resource Guide Vision Community, Family & Relationships Connecting & Contributing in Community Positively Reconnected Empowerment Interdependence Productivity Restoration Identity Higher Power Empowerment Acceptance Healing Forgiveness Transformation Salvation Reliability Purpose Help Support Stability Love Renewal Safety Hope Trust Choice Security Approach FELT NEEDS SPIRITUAL NEEDS LONG-TERM NEEDS 4

FAITH RE-ENTRY RESOURCE CENTERS Faith Re-Entry Resource Centers • County-funded service centers staffed and operated by Collaborative faith partners • Open and welcoming community where re-entry individuals and their families can receive services without judgment or prejudice • Provide additional resources and supports to Collaborative partners to serve reentry individuals and their families – expand service capacity • Refer reentry individuals to Collaborative partners better suited to serve the individual’s needs. • Support the activities of the Work Groups • Work with MHD staff to identify faith based resources and services on a regional basis • Map local Collaborative and non-Collaborative partners – build relationships • Build sphere of community influence and services • Cultivate resource relationships with other service providers • Time Line for Faith Re-Entry Resource Centers • Informal Competitive Process – June 2012 • 8 Proposals submitted • 3 Awards announced: Bible Way Christian Center, Cathedral of Faith, Maranatha Christian Center • November start date 6

Faith Re-Entry Collaborative Today • Over 40 faith organizations currently engaged in Faith Re-Entry Collaborative. • 15-20 Faith Organization membership forms completed. • Over 30 volunteer commitment forms completed • 3 Resource Centers online as of November 2012 – Mission Possible, Good Samaritan and Destiny • Each resource center is in operation with several clients • Resource Centers are actively collecting resources from surrounding churches/faith organizations • 5 work groups actively engaged and growing 8
