  2. Vision In the management of incidents and adverse events, all segments of the Massachusetts healthcare system utilize a jointly derived framework for accountability that is broadly viewed as just.

Past – Priorities Identified • Develop a common language/approach • Encourage all parties to look collaboratively at errors • Make the reporting of errors more effective and integrated • Share lessons learned to minimize errors. • Define a clear policy regarding human error, as well as system error

  4. Present - Objectives • Set of principles to be applied in analyzing the role of systems and individuals in determining accountability • Matrix defining prospective and retrospective responsibilities of all stakeholders, in relation to adverse event • Recommendations to improve process of analysis and investigation
