Clients comes into GP surgery with various symptoms including problems with their diet After further tests the client is diagnosed as diabetic and GP can advise SSD on dietary requirements GP can check whether the client has a MOW service GP can now check on the type of meals that the client is getting. GP Benefits • Supports the overall stategy of integrated care and would be a step in developing the Electronic Health Record • Patient care will become patient centred and more holistic • GP’s will be able to concentrate on patient care rather than phoning SSD • Client information will be available 24 hours rather than just working hours • Less paper flying around so less admin time required at surgeries
