  2. SUBMISSION OF COMMISSIONING PACKAGES • All Commissioning Packages must be submitted to MCRC 4 months prior to the date of appointment via mail. • All Commissioning Packages must have all needed documents prior to mailing to MCRC.

  3. MIDSHIPMAN GROUND COMMISSIONING PACKAGE • This type of commissioning package should be submitted for a midshipman who do not wish to pursue an aviation guarantee. • The student will be commissioned with a MOS of 9901.

  4. NEEDED DOCUMENTS FOR A MIDSHIPMAN GROUND COMMISSIONING PACKAGE • Rough Appointment Acceptance Record (NAVMC 763) • Commanding Officer’s Endorsement • A signed request from the student • BUMED approved physical (that includes the BUMED letter and a copy of the physical and supporting documents that were submitted to BUMED for approval) • Current transcripts • All enlistment contracts (DD Form 4) • NROTC Scholarship or College Program Agreement • Birth Certificate • NROTC Cost Sheets • Security Clearance

MIDSHIPMAN AVIATION COMMISSIONING PACKAGE • This type of commissioning package should be submitted for a midshipman who was granted an aviation guarantee prior to submission of commissioning package. • The midshipman should already have an aviation guarantee letter. • The midshipman will be commissioned with a MOS of 7599 or 7580.

NEEDED DOCUMENTS FOR A MIDSHIPMAN AVIATION COMMISSIONING PACKAGE • Rough Appointment Acceptance Record (NAVMC 763) • Commanding Officer’s Endorsement • A signed request from the student • Aviation Guarantee letter • NOMI approval letter • BUMED approved physical (that includes the BUMED letter and a copy of the physical and supporting documents that were submitted to BUMED for approval) • Current transcripts • All enlistment contracts (DD Form 4) • NROTC Scholarship or College Program Agreement • Birth Certificate • NROTC Cost Sheets • Security Clearance

MIDSHIPMAN GROUND COMMISSIONING PACKAGE with AVIATION REQUEST • This type of commissioning package should be submitted for a senior who has not been granted an aviation guarantee prior to submission of commissioning package and desires a guarantee. • A senior can request for aviation at the time of commissioning ONLY!! • The student will be commissioned with a MOS of 9901, 7599 or 7580. • If the aviation request is approved, the student will not receive any notification from MCRC, he or she will receive commissioning documents with the MOS of 7599 or 7580.

  9. MECEP GROUND COMMISSIONING PACKAGE • This type of commissioning package should be submitted for a MECEP student who do not wish to pursue an aviation guarantee. • The student will be commissioned with a MOS of 9901.

MECEP AVIATION COMMISSIONING PACKAGE • This type of commissioning package should be submitted for a MECEP student who was granted an aviation guarantee prior to submission of commissioning package. • The MECEP student should already have an aviation guarantee letter. • The MECEP student will be commissioned with a MOS of 7599 or 7580.

MECEP GROUND COMMISSIONING PACKAGE with AVIATION REQUEST • This type of commissioning package should be submitted for a senior MECEP student who has not been granted an aviation guarantee prior to the submission of commissioning package and desires a guarantee. • A senior can request an aviation guarantee at the time of commissioning ONLY!! • The student will be commissioned with a MOS of 9901, 7599 or 7580. • If the aviation request is approved, the MECEP student will not receive any notification from MCRC, he or she will receive commissioning documents with the MOS of 7599 or 7580.

INSTRUCTIONS FOR COMPLETING THE NAVMC 763 • Block 1- Name • Block 2- SSN • Block 3- Date of Birth • Block 4- HOR (Block 3 from DD Form 4) • Block 6- Place of Birth (city and state of birth on birth certificate) • Block 7- 9901 (if going ground); 7599 or 7580 (if going air, whether requested prior to submission of commission package or during submission of commission package • Block 9- US (always) • Block 9A- CA (always) • Block 10- Sex (M for male or F for female) • Block 14 and 14a- Religion Preference (see below) Religion code (see below) 01 = No Preference 10 = Baptist 13 = Christian 62 = Roman Catholic 70 = Protestant 99 = Unknown
