3. SUBMISSION OF COMMISSIONING PACKAGES Due to MCRC 120 days prior to the date of appointment via mail. (NO PDF FILES OR FAXES UNLESS REQUESTED BY MCRC) Due the 1st day of the deadline submission month. Must request extension in writing of any packages that cannot meet the given deadline. Must include all required documents prior to mailing to MCRC. Packages not received 120 days prior to requested commission date will be rescheduled. 5. MIDSHIPMAN GROUND COMMISSIONING PACKAGE This type of commissioning package should be submitted for midshipman who do not wish to pursue an aviation guarantee. 7. MIDSHIPMAN AVIATION COMMISSIONING PACKAGE This type of commissioning package should be submitted for a midshipman who was granted an aviation guarantee prior to submission of commissioning package. The midshipman should already have a MCRC aviation guarantee letter. The midshipman will be commissioned with a MOS of 7599 or 7578.

A senior can request for aviation at the time of commissioning ONLY!! The student will be commissioned with a MOS of 8001, 7599 or 7578. If the aviation request is approved, the student will receive notification from MCRC.

COMMON DISCREPENCIES RFA PACKAGES ARE NOT TO BE EMAILED OR FAXED TO MCRC PACKAGES ARE STILL BEING SUBMITTED AS GROUND W/AVIATION REQUEST ALTHOUGH FY10 AVIATION GUARANTEES/REQUEST HAS BEEN CLOSED OUT. Tattoo Screening Forms are not submitted in the RFA packages because of the misconception that it’s only to be included if the MIDN has a tattoo/new tattoo; Tattoo Screening Forms/SOU are to be included in EVERY RFA package. Marital Status not included on the student’s letter. Birth Certificate requests used as a substitute for the Birth Certificate-YOU MUST SUBMIT THE BIRTH CERTIFICATE/BIRTH VERIFICATION.

24. COMMON DISCREPANCIES The “Signed NAVMC 763” is being used instead of the “Rough NAVMC 763” in the RFA packages -THE ROUGH NAVMC 763 IS TO BE USED AT ALL TIMES IN THE RFA PACKAGES-IF THE PACKAGES ARE SUBMITTED WITH THE “SIGNED NAVMC 763” THE UNITS WILL BE CONTACTED TO RESUBMIT THE CORRECT FORM Units are submitting EITHER the DD Form 2808 or the DD Form 2807-BOTH THE DD FORM 2808 AND DD FORM 2807 ARE REQUIRED FOR THE RFA PACKAGE. ***THE BEST WAY TO LOWER THE DISCREPANCIES OF YOUR SUBMITTED RFA PACKAGES, IS TO USE THE EXAMPLES ON THE WEBSITE {i.e. The Checklist and/or The RFA Sample Packages} THE WEBSITE HAS BEEN UPDATED WITH THE LATEST INFORMATION THAT MCRC REQUIRES FOR ALL PACKAGE SUBMISSION***
