  3. Purpose: Why so important? • Without assessment, my efforts could not be as effective. • As school counselors, we have a good idea of our school’s climate, but cannot possibly know EVERYTHING.

Individual Counseling • Documentation System • Analyzing session totals, referral data • Sample (Handout Packet) • Behavior Referrals • Are formal “write-up’s” decreasing for students you work with? • SWIS: School-Wide Information System • My district uses this. It provides monthly graphs and reports based on formal behavior referrals. • Session Data • Sample Activities (Handout Packet)

Small Group Counseling • Documentation System • Similar to Individual Counseling system • Sample (Handout Packet) • Session Data • Sample Activity (Handout Packet) • Behavior Referrals • When applicable, are formal “write-up’s” decreasing? • Fewer issues/referrals (individual counseling) • Qualitative Data • Student, Teacher/Staff, and Parent Response
