Founding In 2001, the American Heart Association, the Association of State and Territorial Health Officials, and the Centers for Disease Control and Prevention founded the National Forum to provide leadership for implementation of A Public Health Action Plan to Prevent Heart Disease and Stroke, a national plan to chart a course for the prevention of heart disease and stroke.

  9. 1998 - 2005 Laying the Ground work to Setting Priorities • 1998 - Congress awards appropriation to CDC, establishing a state heart disease and stroke prevention program • 2003 - A Public Health Action Plan to Prevent Heart Disease and Stroke is published • See page 49 in the Annual Meeting program book for a full list

  10. 2006 – 2009Action Plan to Affect • 2006 - Dr. Darwin Labarthe, 1st National Forum Chair • 2007 - Essential Features of a Surveillance System to Support Prevention and Management of Heart Disease and Stroke is published in January 2, 2007 issue of Circulation • 2008 - Update to the Action Plan • 2009 - National Forum convened the Economics of Cardiovascular Disease: Defining the Research Agenda, May 14, 2009 • 2009 - National Forum convened a Global Summit on Education and Training in Heart Disease and Stroke Prevention, October 15-16, 2009 • 2009 -National Forum hires first Executive Director

  11. 2010 and BeyondCreating the Will – Ready to Fly • 2010 - Establishment of 501c3 organization • 2010 - Inaugural Leadership Roundtable at AHA Scientific Sessions • 2011 - ”Health Economics of Cardiovascular Disease: Defining the Research Agenda” was published in CVD Prevention and Control, September 2011 • 2011- National Forum launches 2020 Goal and 2011 - Strategic Priorities • 2012 - Policy Depot Launch • 2012 - National Forum establishes its office in Washington DC • See page 49 in the Annual Meeting program book for a full list

  12. 5 Essential Components ENGAGING IN REGIONAL AND GLOBAL PARTNERSHIPS ADVANCING POLICY TAKING ACTION STRENGTHENING CAPACITY EVALUATING IMPACT “Implementation…must assure integration of all five components in a coordinated approach that recognizes and strengthens the potential linkages among them. Such integration and coordination are critical to effective implementation.”

  15. Strategies to Reduce Sodium Intake in the US “Americans consume unhealthy amounts of sodium in their food, far exceeding public health recommendations. Consuming too much sodium increases the risk for high blood pressure, a serious health condition that is avoidable and can lead to a variety of diseases. Analysts estimate that population-wide reductions in sodium could prevent more than 100,000 deaths annually.” - Institute of Medicine April 20, 2010
