  3. IMS Fall Sports – Middle School Boy’s Fall Sports: Girl’s Fall Sports: Basketball Cheerleading Cross Country Field Hockey (2 teams) Soccer (2 teams) Slow Pitch Softball • Cross Country • Football • In-Line Hockey ( 7 – 9) • Soccer (2 teams)

  4. Physical Forms • Please visit the athletics website: http://www.northallegheny.org/site/Default.aspx?PageID=772 • All forms must be sent directly to the athletic office. (no later than one week prior to start of season) • Coaches are not permitted to accept physical exam forms. • Late physicals may result in the student not being able to participate in practice or tryouts. • If you have not had a physical this year you will need to get a physical before the start of your season. • Students participating in multiple sports within the same year need to turn in a Section 7 form for 2nd and 3rd sport played. • Students who have sustained a serious injury need to Complete a Section 7 and 8 form.

  5. Impact Testing • North Allegheny Athletic Department mandates that all students participating in athletics complete the UPMC ImPACT Concussion Baseline Testing Program. • Testing is mandatory for every student-athlete entering grade 7th and 9th. • Students entering 8th do not need to take an ImPACT test for 2012 – 2013 school year unless they did not take the test last year.

Summer Recreation Schedule • North Allegheny is offering Summer Rec for the following sports: Football (7th – 12th grade), Boys and Girls Cross Country (7th – 12th grade), Girls Soccer (3rd – 12th grade), Field Hockey (4th – 12th grade), Girls Volleyball (4th – 12th grade) and Boys Volleyball (2nd – 12th grade). • Please visit the North Allegheny Athletic Website for a summer recreation form
