ACSHIC WEBMD REWARDS – Tier 1TO GET STARTED • After completing the Wellness Profile (and to receive proper credit for Tier 1), remember to sign up and show progression for a Digital Health Assistant program. • These can be accessed by hovering over the “Healthy Living” blue bar at the top of the screen then clicking on “My Health Assistant”. You can also access these from the main members page by clicking on the “Health & Wellness” button, click on “Symptom Checker”, then scroll over the “Healthy Living” tab and select “My Health Assistant”

  9. ACSHIC Webmd rewards – TIER 1 • Below is a sample of what the Digital Health Assistant screen looks like • REMINDER-to earn the full credit for Tier 1, you need to set a Digital Health Assistant goal and show progress towards your weekly plan

Points can be achieved through Highmark programs, your School District sponsored programs or even community based programs through Highmark REACHING TIER 2 WILL EARN YOU THE SECOND $25 GIFT CARD

ACSHIC WEBMD REWARDS – TIER 2 To submit points that you have earned, go to www.highmarkbcbs.com, sign in and click on the Rewards Program WebMD link located under Programs on the right side of your members page. Here you can view your history of points earns and enter self-reported activities. When entering an activity ALWAYS click SUBMIT to ensure you receive credit.

Activities to earn your points… • Wellness Profile The comprehensive health risk assessment helps members understand their health status and gives you a personalized action plan to improve or maintain your current health. • Personal Health Record With the secure online personal health record (PHR), store and access health information anytime, anywhere. Grant access to health care providers and family members to facilitate treatment, for example, in emergencies. Track medications, immunizations, medical exams, treatment history and other health information for yourself or other family members. • Health Trackers You can choose from 24 different trackers to record and view your progress in key health measures such as blood pressure, blood glucose, cholesterol, and weight. • Health Management Centers These proven resources help members manage health issues, such as exercise, weight management, nutrition and stress. • Digital Health Assistant Programs Learn about programs offering real life solutions to everyday wellness challenges. Topics include exercise, nutrition, stress management, weight management and emotional health. • Symptom Checker This unique interactive feature makes it easy to learn what to do about symptoms and when to contact a doctor. Click on the part of the human body graphic where your discomfort or symptoms exist. • Health Education & Information With news articles, a health library, condition guides, e-newsletters, videos, and links to relevant health information, research a specific health topic or get the latest info on health trends.

ACSHIC Webmd rewards - TIER 3REWARDS BONUS • To complete Tier 3 (Bonus Tier), earn an additional 50 points (150 points total including Tier 1 and Tier 2). • Points can be achieved through preventive exams, Highmark programs, your School District sponsored programs or even community based programs through Highmark • Completing Tier 3 will earn your 3rd, $25 Gift Card • You have until April 30, 2013 to reach this goal

After achieving an additional 50 points (100 total including Tier 1), you will be eligible for your 2nd, $25 Gift Card. • Tier 3-BONUS-If you accumulate another 50 points (150 total including Tier 1 and 2), you will be eligible for your 3rd, $25 Gift Card PLEASE NOTE: YOU MUST COMPLETE TIER 1 FIRST TO BE ELIGIBLE FOR THE TIER 2 and TIER 3 INCENTIVE.
