  4. OverviewTexas Medical Home Toolkit • Developed by the Texas Parent to Parent Program, Medical Home Program, and the Texas Medical Home Workgroup and the Texas Department of State Health Services-Children with Special Health Care Needs Services Program

  5. Overview Texas Medical Home Toolkit • Information for families on the concept of Medical Home and how to get one setup for your child or clients who have special health care needs. Includes: information to my child’s doctors about the help I need from him /her, and information on how to provide support to families who would like to have a medical home.

  6. Family to Family Health Education and Information Center • Washington Family to Family Network (WFFN) reviewed the Medical Home Checklist from Texas • Decision was made to develop a one page bulletin for using the Texas model as a guide but it would be Washington’s check list and link to our resources and supports for families • The Medical Home Checklist from Texas was reviewed by the WFFN and the 9 member advisory board for input

Next steps and dissemination • Finalize the draft with approval from WFFN and the Family to Family Advisory Board • The bulletin will be disseminated through the Family to Family list-serve, website, WFFN networks and at conferences and trainings . • Link with the Washington Medical Home Leadership website
