Welcome back and why we are here today • As part of our on-going commitment to keep our patients, carers and the public informed about, and involved with, local NHS developments • To explain to you how we are continuing to make improvements to services offering urgent care • To understand why we need these changes, and also how we together can ensure these changes not only sound good, but improve the patient / carer experience through improved service quality • To start a discussion with you around our next years intentions • To answer your CCG related questions

About the current Hillingdon Urgent Care Centre (UCC) • Located at the entrance of the Hillingdon Hospital Accident and Emergency department • It is not a new service, Hillingdon UCC has been in operation since 2007.Open 9am – midnight, 7 days a week • Currently provided by Harmoni (Now part of Care UK) • Provides a walk-in service for patients attending the Accident and Emergency department with minor illnesses and injuries • Designed to free up the Accident and Emergency department to allow staff to treat patients presenting with critical to life threatening conditions……

  4. * 30,748 patients treated in 2012 /13 * • Common uses amongst adults: • Back pain (muscular) • Infections: Ear, throat, chest and skin • Pregnancy related conditions (e.g. Bleeds) • Head, chest and hand injuries (mild) • Abdominal pains • Labyrinthitis (Ear infection) • Common uses amongst children: • Infections: Chest, ear, nose and throat, • Gastroenteritis • Head injuries

  5. The case for change….. Align with recommended standards of care Provide 24/7 Urgent Care Improve patient / Carer experience Improve the quality of care locally Join up urgent care services Public Patient Carer Better health outcomes Support patients and carers access the right care Enable positive re-direction Reduce A&E attendance for minor conditions Access to diagnostics

The search for the right provider • Procurement process ( March – July 2013) • Panel: External GP, Hillingdon GP (Dr Mitch Garsin), Health Watch, Trevor Begg (Governing Body lay member), Subject matter experts (e.g. Commissioning, Estates, IT, Human Resources) • Began with 11 applications, of which 6 applicants were invited to tender for the service. (March – May) • Evaluation process included: Review of bids, presentation and interview. (June) • Selected and notified preferred provider (July)

What will the expanded service look and feel like? 24 hours a day, 7 days a week UCC Reception & Navigation Treating GPs & Nurses in UCC • GP consultation – Minor illness, more complex conditions, selected patient groups (e.g. pregnant, elderly) • Nurse Practitioner consultation – most minor illnesses and injuries • HCA – dressings, suture removal etc. • Appropriate use of diagnostics • Reception: • Books patient • Provide GP practice registration support Home Walk In General Practice / Community Patients 111 • Navigator Nurse • Streams to: • UCC GP/ENP • Diagnostics • Further triage • ED • Redirection Emergency Department GP Ambulance

Understanding the need to address GP access • Translating our Out of Hospital Strategy ….. • Looking at how we can share expertise and best practice • Identifying what is needed and how we can respond positively to the needs of patients in a primary care setting • Over the coming months we will….. • Work even more closely with patients, carers and the public to identify how and what services we should commission and move closer to home • Monitor patient satisfaction survey data, complaints and comments more closely to ensure our vision is your reality

But improving access also lies with you…. • GP appointments: • Attend your appointment, or inform us of a cancellation • Share this message with neighbours, friends and family • Ask for / consider having a telephone appointment with your GP • When your GP is not available: • NHS 111 • Seek advice from your local pharmacy • Minor injuries unit at Mount Vernon hospital • Complaints, Compliments, Comments: • At your GP surgery / NHS England • Hillingdon CCG website www.hillingdonccg.nhs.uk • Hillingdon Healthwatch

It always was, and still remains your NHS…. • No decision about you, without you….get involved • Whether you only have 10 minutes or 10 hours … • Now: • HCCG Equality Objectives public consultation • August / September: • Commissioning intentions (surveys, discussion groups) • Joint survey with HCCG, CNWL, LBH and THH • September and beyond: • Service re-design • Out of Hospital Strategy
