  2. Employment Forms to Complete Through MYLSU • Once you have been hired you will receive an email with a link to your Employee Information Page (EIP). The EIP will contain several required employment documents. • Form I9 • Work Permit * Allow International Services at least 5 days to process. • Alien Tax Information Form • W-4 and L-4 • Direct Deposit Form • Other University required forms

Employment Forms to Complete Through MYLSU……… • Work Permit • F-1/J-1 student workers and graduate assistants must have an unexpired International Services (I.S.) work authorization document in order to work. Click on the Work Permit link to access and complete your portion of the form. It will be sent to I.S. for approval. If there are any problems you will receive an email from I.S. indicating what action needs to be taken. Only begin working on or after the effective date of the authorization. • Form I9 • Select the link from the EIP and complete the top portion of the form. The form will be sent to I.S. for approval.

  6. Employment Forms to Complete Through MYlSU....... • W-4 and L-4 • These forms are completed online through MYLSU and determine your tax withholding amounts. As a “non-resident alien” you must claim “single” (regardless of martial status), and one allowance (exemption) for yourself. There are exceptions for residents of Canada, Mexico, India and S. Korea. • Direct Deposit Form • Direct Deposit is mandatory for payroll checks. Complete the Direct Deposit form through MYLSU as part of your employment paperwork to authorize LSU to deposit your payroll to the U.S. bank account of your choice. • Alien Tax Information Form • Required by the Payroll Office to monitor residency status for tax • purposes.

Income Tax Treaties • See Tax Treaty Facts for listing of Treaty Countries. http://www.fas.lsu.edu/AcctServices/pay/forms/facts.pdf • Eligibility dependent on prior residency and primary purpose for visit. • Must be renewed each calendar year. • Forms can be obtained in the Payroll office at 204 Thomas Boyd Hall or the Payroll website; http://www.fas.lsu.edu/AcctServices/pay/treaties.html • Forms 8233 and the Attachment to 8233 must be turned into Payroll to be approved and implemented.

Taxpayer ID Numbers • Social Security Number (SSN) • If you do not have a U.S. Social Security number but an assigned temporary number (beginning with 999), you will not be paid until you receive a U.S. Social Security Number. The SSA Office is located at 5455 Bankers Ave., Baton Rouge, LA 70808. Bring your original documents, including SS-5 Social Security Application form, passport, visa, I-20, I-94, and the DSO/Department hiring letter with original signatures to apply for a SSN. For more information, please see: http://international.lsu.edu/IS/current/socialsecurity.php Check with the International Cultural Center for help with your transportation to the SSA office. • Once you receive your Social Security card bring the card to the Payroll Office so that your records can be updated and payment can be processed. • Individual taxpayer Identification Number (ITIN) • Students who receive a taxable scholarship but are not employed and not eligible for a Social Security Card need an ITIN. This number allows LSU to report scholarships to the IRS at the end of the year and allows you to file your tax returns. You may apply for the number in the Payroll Office. Bring passport, visa and I-20 / DS-2019 to complete the necessary forms.

  11. Visa Type and FICA Taxes • F1/J1 students are exempt from paying Medicare and Social Security taxes up to five calendar years. In the sixth year, you will be considered a “resident alien” for tax purposes since you will then pass the substantial presence test. As a resident alien, Medicare and Social Security taxes are required to be withheld during the summer if working and not enrolled in summer school. • If you are on F1/J1 visa and have not met the substantial presence test, and Medicare and/or Social Security taxes are withheld from your payroll check, you should notify the Payroll Office, so that your records can be corrected and taxes refunded.

  12. Filing income tax returns • Tax Documents Issued by LSU: - W-2 Form; from Payroll by January 31st. Reports taxable earnings for the previous calendar year. • 1042-S Form; from Accounts Payable by March 15th. Reports tax treaty earnings or any taxable scholarship income for the previous calendar year. • 1098-T Form; from Bursar by January 31st. Reports amounts paid for tuition. Non-resident aliens for tax purposes will not use this form.

Provides free tax help to internationals. • Glacier Tax Prep Software by Arctic International https://www.glaciertax.com/ Cost was $34.00 for tax year 2013 Only available for Fed Tax, not State Tax.
