COAG Indigenous Reform Agenda targets for Closing the Gap • To close the gap in life expectancy within a generation • To halve the gap in mortality rates for Indigenous children under five within a decade • To ensure all Indigenous four years olds in remote communities have access to early childhood education within five years • To halve the gap in reading, writing and numeracy achievements for Indigenous children within a decade • To halve the gap for Indigenous students in year 12 attainment or equivalent attainment rates by 2020 • To halve the gap in employment outcomes between Indigenous and non-Indigenous Australians within a decade.

The Indigenous Health NPA (IHNP)five priority areas Funding of approximately $1.6 billion over 4 years: • Commonwealth: $805.5 million • States and Territories: $771 million Five Priority Areas • Tackling smoking • Primary health care services that can deliver • Fixing the gaps and improving the patient journey • Providing a healthy transition to adulthood; and • Making Indigenous health everyone’s business. The Commonwealth will contribute to the first three and state and territory governments will contribute to all five priorities.

Three Broad Strategies The ICDP package has three broad and integrated strategies targeting closing the life expectancy gap between Aboriginal and non-Aboriginal Australians • Tackling chronic disease risk factors ($161m) • Improving chronic disease management and ($474m) follow-up care • Workforce expansion and support ($171m)

  5. Workforce Expansion • 80 Project Officers & 80 Indigenous Outreach Workers located in Divisions of General Practice • 1 Project Officer each for GPNSW & the AH&MRC • 80 Indigenous Outreach Workers in the Community Control Sector • 105 Healthy Lifestyle Workers and training for 400 existing nurses or AHWs to deliver chronic disease management self-management programs • National Tobacco Coordinator as well as Regional Tobacco Coordinators and Tobacco Action Workers

DGP Project Officers • Support to mainstream primary care providers and Aboriginal Medical services in all ICDP measures • Provide mentoring support to IOWs • Improve capacity of primary care to deliver culturally appropriate health care for Aboriginal people • Establish & maintain linkages between AMSs and general practice • Have a mentoring role for IOWs

  7. Aboriginal Outreach Workers Both in AMSs and DGPs Key roles • Community liaison • Administration & Support to IHPO • Provide practical assistance • Provide feedback on access

  8. Healthy Lifestyle Workers • Focus on poor nutrition and physical increasing activity • 105 nationally • First line advice and referral issues • Can work across home, community meeting places, sporting events, schools or community stores • Training for nurses & AHWs to deliver CD self-management programs

  9. Tackling Tobacco – National Workforce Parts of this measure are: • National Tobacco Coordinator • Regional Tobacco Coordinators • Tobacco Action Workers • Social marketing

  10. MSOAP ICD & CCSS • Expansion of MSOAP services specifically to increased access to specialist services for Aboriginal people • Coordinated Care & Supplementary Services funding for “brokerage” type services
