West Derby Golf Club Autumn Seniors Thursday 6th September 2012  Competition Entry form Thursday 6th September 2012 Men’s Open Seniors  (Minimum Age 55) Fourball Betterball Stableford  Entry Fee £30.00 per pair (includes Ball Sweep) All competitors will be entitled to a discounted green fee for a two ball price of £35.00 or four ball price of £60.00 This offer is available until 31st October 2012.

  2. # • CONDITIONS & REGULATIONS • Entries to reach the Manager, West Derby Golf Club, Yew Tree Lane, Liverpool L12 9HQ, by 1st September 2012. • A stamped self addressed envelope and the appropriate Entry Fee must accompany the entry form otherwise entries will not necessarily be considered or acknowledged. • Tee times and Markers will be arranged upon application. The order and starting time will be posted to each first named competitor. Pairs wishing to nominate specific playing partners should notify the Club accordingly when entering. After July 1st entrance fees will not be refunded. • In the event of a tie (or ties) the rules of the Lancashire Union of Golf Clubs shall apply. • Each entrant must be a member of a recognised Golf Club & hold a current handicap. The Committee reserves the right to refuse any entry without assigning a reason for its action. All disputes shall be referred pending a decision, which shall be final. • The Committee reserves the right to accept late entries but only if suitable times are available • Notes: • Full catering facilities are available throughout the day. • Meals should be booked on the day, prior to commencing play. • Morning coffee will be available from 8:00am. • The Prize Presentation is scheduled for 8:30pm. • (Winners may be expected to produce handicap certificates) • Smart casual wear is permitted in the dining room up to 7:00pm, after which time jacket, shirt and tie must be worn. • Competitors must not enter the lounge/dining room wearing clothes in which they have just played golf. • Karen Buck • Manager ENTRY FORM Competition:……………………………………………………….
