FOSSE AXILLAIRE PAROI INFERIEURE Aponévrotique et cutanée Concave dans le sens transversal et antéro-postérieur (creux de l’aisselle) 3 couches ( peau avec gl sudoripares et poils, tissu sous-cutané, fascia axillaire) Fascia axillaire: aponévrose tendue entre bord inf grd pect, grd dorsal, grd rond, grd dentelé et coraco-brachial (orifice pour paquet vx-nerfs)

  11. FOSSE AXILLAIRE SOMMET Orifice ostéo-fibreux: entre clavicule, 1ère côte, apophyse coracoïde et insertions ligamentaires Communication entre fosse axillaire et creux supra-claviculaire (pédicule axillaire)

FOSSE AXILLAIRE PAQUET VASCULO-NERVEUX • Plexus brachial: En dehors artère 3 troncs secondaires et branches terminales: • Tronc II antéro-externe -> nerf musculo-cutané et racine externe nerf médian • Tronc II antéro-interne -> racine interne nerf médian et nerfs ulnaire, brachial cutané interne, accessoire du brachial cutané interne • Tronc II postérieur -> nerf radial et nerf circonflexe

FOSSE AXILLAIRE PAQUET VASCULO-NERVEUX • Rapports: Paquet perpendiculaire au petit pectoral • Au dessus petit pectoral (sur paroi interne): artère centrale, troncs II externes et veine en arr • Derrière petit pectoral (contre paroi ant): artère centrale, branches terminales plexus autour (V médian en av, musculo-cut en dh, nerf ulnaire entre artère et veine, radio-circonflexe post), veine en dd

FOSSE AXILLAIRE LYMPHATIQUES ET GGS AXILLAIRES Drainage du mb sup, de l’hémiparoi thorax et abdo et sein homolat+++ (cancer) 5 groupes de ggs: • thoracique latéral ou mammaire ext (<= thorax, abdo, sein) • Brachial (veine axillaire; <= mb sup) • Sub-scapulaire (<= tronc post) • Intermédiaire (vx axillaires; <= 3 groupes précédents) • Sub-claviculaire (sommet fosse)

  19. FOSSE AXILLAIRE LYMPHATIQUES ET GGS AXILLAIRES Réunion en un tronc collecteur unique: tronc sub-clavier -> lymphatiques supra-claviculaires
