Web Addresses • E-mail from research@connectxtherapy.com with links to: • (1) Treatment Plan and Side-Effects Data Collection • Clinician completes the survey at each visit • https://www.assessmentcenter.net/ac1/Assessments/Burke_ConnecTX_Pain_Clinician2 • (2)Data Collection Survey that the Patient Completes • The patient completes the survey at their first visit (baseline) and at their last visit (the completion of care). • Pre-Post Assessment • https://www.assessmentcenter.net/ac1/Assessments/Burke_ConnecTX_Pain_Patient

  5. Set Up: Electronic survey Data CollectionTreatment Plan and Side-Effects Using a Computer (Desktop or Laptop)

  6. Open the e-mail Link to: • 1. Study Identified by your last name (Prefix) • 2. Suffix indicates Clinician Survey • 3. Click Add to Favorites to save the address • 4. Close the Welcome Page Window

  7. Verify Link was SAVED to Favorites • Go to Web Browser • Go to your Favorites Task Bar • Click on the Clinician Survey Link • Welcome Page should Open

  9. Open the e-mail link: https://www.assessmentcenter.net/ac1/Assessments/Burke_ConnecTX_Pain_Patient Welcome Screen Provides Background Information for Patient Click Add to Favorites Then Close Welcome Screen

  10. Verify Link was SAVED to Favorites • Go to Web Browser • Go to your Favorites Task Bar • Click on the Patient Survey Link • Welcome Page should Open

Bookmark Clinician Survey • Click on Safari • Type Clinician Survey Address into “Go to this address” • https://www.assessmentcenter.net/ac1/Assessments/Burke_ConnecTX_Pain_Clinician2 • Clinician Survey Opens • Save the Clinician Survey as a Bookmark • Close the Clinician Survey • Go to the Bookmarks • Click Edit the Bookmarks • Select Clinic Survey Bookmark • Change the web address to your Clinic Survey Address • https://www.assessmentcenter.net/ac1/Assessments/Burke_ConnecTX_Pain_Clinician2

  13. Open Clinician Survey on iPad • Click on Safari • Click on Bookmarks • Click on Clinic Survey Bookmark • Welcome Page should Open

Bookmark Patient Survey • Click on Safari • Type Patient Survey Address into “Go to this address” • https://www.assessmentcenter.net/ac1/Assessments/Burke_ConnecTX_Pain_Patient • Patient Survey Opens • Save the Patient Survey as a Bookmark • Close the Patient Survey • Go to the Bookmarks • Click Edit the Bookmarks • Select Patient Survey Bookmark • Change the web address to your Patient Survey Address • https://www.assessmentcenter.net/ac1/Assessments/Burke_ConnecTX_Pain_Patient

  15. Open Patient Survey on iPad • Click on Safari • Click on Bookmarks • Click on Patient Survey Bookmark • Welcome Page should Open

  17. Inclusion Criteria • Pain symptoms in patients with musculoskeletal disorders related to the spine • Age Range: 20 to 70 years old regardless of gender, race or ethnicity • Treatment plan includes ConnecTX™ Therapy with a goal to reduce pain symptoms

  18. Sample Size • 5 patients per participating clinician

Demonstration of Clinician Survey • Use the below address on the Internet • https://www.assessmentcenter.net/ac1/Assessments/Burke_ConnecTX_Pain_Clinician2 • If using Computer or Laptop • Cut and paste the address into the web browser’s address bar • If using an iPad, • Within Safari, type the address into “Go to this address”

  21. Welcome Page We are not using the Login and Password feature Just click Start

  22. Other Patient Information • Type in Age • Click Gender • Then click Continue

  23. Patient Name • Type in Patient’s First Name • Then click Continue

  24. Summary Information Screens Screen 1 Just Click Begin Screen 2: Just Click Continue

  25. Answer Survey Questions • Dr. Hunter Mollin will review the questions in class. • Contact research@connectxtherapy.com with any additional questions.
