MEDICAL BENEFITS | cost effective peace of mind It is important to note that while the plan may not require you to use a primary care physician, it remains your responsibility to make sure you are using an in-network provider in order to enjoy the benefits of the Plan’s in-network benefit schedule.

MEDICAL | Key Benefits • $20 co-pay / office visit (in-network provider) • 90% coverage for network expenses, after the individual annual deductible of $200/$600 family, and 70% coverage for non-network expenses after the individual annual deductible of $400/$1,200 family • Network annual maximum out-of-pocket is $600 ($800 including deductible) per person or $1,800 ($2,400 including deductible) per family • Non-network annual maximum out-of-pocket is $1,800 ($2,200 including deductible) per person or $5,400 ($6,600 including deductible) per family • Working spouse/domestic partner provision

MEDICAL | Key Benefits • $100 co-pay for Emergency Room • $35 co-pay for Urgent Care Facility • $20 co-pay for Outpatient Physical Therapy, with an annual maximum benefit of 40 visits • $20 co-pay for Outpatient Chiropractic Care, with an annual maximum benefit of 40 visits • $20 co-pay for Outpatient Mental Health/Substance Abuse, with an annual maximum benefit of 50 visits • $20 co-pay for Well Child Care Visits • $20 co-pay for Routine GYN Visit • 100% coverage for Preventive Care for Employee and dependents age 12 and older – Exam and all tests required due to age and family history • $20 for Routine Eye Exam – 1 exam every two years

  9. Here's why you get the most from your plan when you choose network providers: $20 office visit co-pays The plan pays 90% after you pay your medical deductible. The plan pays 100% for covered services when you see a network provider, and you don’t have to pay a deductible or coinsurance. A network provider won’t bill you for more than the allowed amount. You won’t have to file a claim if the plan is your primary coverage. Why use a Network Provider?

  10. Non-network providers are providers who do not contract with the United Healthcare networks. The plan pays only 70% of the allowed amount to non-network providers. The allowed amount is the amount network providers agree to accept as payment in full. Non-network providers can charge more than the allowed amount, so you will pay 30% of the allowed amount plus anyamount a non-network provider charges above the plan’s allowed amount. Benefit Amount: 1 x salary to a maximum of $750,000 without medical underwriting or up to $1,000,000 with medical underwriting. IMG pays 100% of the cost of this coverage

  17. LIFE BENEFITS | there when you need it Optional Life Insurance – provided by MetLife If you want additional protection for you and your family, you can buy: 31 days to enroll, after 31 days subject to medical underwriting

YOUR BENEFITS | core benefit solutions that work for you Long Term Disability – provided by MetLife • Eligible employees are enrolled on the first of the month following 30 days of employment at no cost to the employee • Covers 50% of monthly base salary with a maximum monthly benefit of $5,000 • Benefit payable after 180 days of disability until no longer disabled or the at most to age 65 • Partial disability benefit payable if employee experiences at least a 20% reduction in pay after a period of disability IMG pays 100% of the cost of this coverage

  20. FINANCIAL BENEFITS | there when you need it Flexible Reimbursement Accounts – Administered by UHC A Flexible Reimbursement Account works like a savings account. Upon enrollment, a pre-tax payroll deduction is deposited into your Flex Spending and/or Dependent Care Assistance Account each pay period. Unclaimed balances in a Flexible Reimbursement Account at the end of the plan year are forfeited and not returned to you. Please give careful thought to your election . Be conservative in your estimated annual expenses. Contributions are automatically deducted from your paycheck on a pre-tax basis.

FINANCIAL BENEFITS | there when you need it 401(k) Retirement Plan The IMG 401(k) Plan makes saving for retirement easy and painless. • Employees are eligible on the 1st day of the month following 12 months and 1,000 hours of service. • IMG will contribute 3% of your earnings as a non-elective contribution to your account. You receive 3% regardless of your participation! • IMG contributions are subject to immediate and full vesting! • You can contribute up to $17,000 in 2012 to the plan on a pre-tax or after-tax basis.
