病例分析一. 东莞东华医院放射科 朱刚明. 概况. 患者，男， 33 岁 ； 主诉：左上腹疼痛 5 年，加重 4 天 ； 现病史：患者于 5 年前无明显诱因出现左下腹不适，偶有疼痛。近 1 年来腹痛加剧，服用中药（具体不详）后症状有所缓解。 4 天前腹痛加重，于外院就诊时 B 超发现左下腹包块，遂转入本院。 既往史、个人史、家族史：无特殊. 体格检查. 体温： 37°C ， 心率： 65 次 / 分， 呼吸： 20 次 / 分， 血压： 150/85mmHg （ H ）

  1. 病例分析一 东莞东华医院放射科 朱刚明

  2. 概况 患者，男，33岁 ； 主诉：左上腹疼痛5年，加重4天 ； 现病史：患者于5年前无明显诱因出现左下腹不适，偶有疼痛。近1年来腹痛加剧，服用中药（具体不详）后症状有所缓解。4天前腹痛加重，于外院就诊时B超发现左下腹包块，遂转入本院。 既往史、个人史、家族史：无特殊

  3. 体格检查 体温： 37°C， 心率：65次/分， 呼吸：20次/分， 血压：150/85mmHg （H） 左下腹可扪及一包块，范围约9×10cm，活动度可，压痛（+），反跳痛（-）。

  4. 生化检查 WBC 6.3G/L RBC 7.158T/L 血红蛋白 149.2q/L 淋巴细胞百分数 25.57% 血皮质醇（上午8时）267nmol/L 尿儿茶酚胺 154.3nmol/L。

  5. 超声检查 • 左肾集合系统分离10mm。左上腹可见一范围87x65mm的低回声包块，形态不规则，边界清。

  6. CT平扫
